Causal Links Between Type 2 Diabetes and Hypertension and TCM Intervention Prediction:a Comorbidity Perspective

目的 采用孟德尔随机化(Mendelian randomization,MR)方法探讨2型糖尿病与高血压病之间的因果关系,并预测具有调控2型糖尿病与高血压病潜力的中药.方法 从公开数据库获取2型糖尿病与高血压病的全基因组关联研究汇总数据,利用R语言中的Two Sample MR软件包进行MR分析,以逆方差加权法作为评估遗传因果效应的主要方法.对工具变量邻近基因进行功能富集分析,以探索相关信号通路.结合Coremine数据库(https://coremine.com/medical/)预测对工具变量邻近基因具有潜在调控作用的中药,并统计其性味、归经及功效信息.结果 MR分析显示,2型糖尿病与高血压病发病风险升高相关.工具变量邻近基因主要富集于磷脂酰肌醇-3-激酶(phosphatidylinositol-3-hydroxykinase,PI3K)/蛋白激酶B(protein kinase B,Akt)、鼠肉瘤(rat sarcoma,Ras)和丝裂原活化蛋白激酶(mitogen-activated protein kinase,MAPK)以及内分泌抵抗等信号通路.中药四气以寒为主,辅以温、平;五味以苦为主,辅以甘、辛;归经首重于肝,其次为肺、脾、胃、肾;功效以清热为先,补虚次之,同时涵盖活血、理气、祛风湿、化痰止咳平喘、利水渗湿等.具有这些高频特征的代表性中药包括丹参、人参、黄连、黄芩、郁金、姜黄、黄芪等.结论 MR分析发现,2型糖尿病可能增加高血压病的发病风险,其机制可能通过PI3K/Akt、MAPK、Ras等信号通路介导高血压病的发生与发展.潜在调控中药主要包括清热药、补虚药、化痰止咳药和理气药等,为中医药防治糖尿病合并高血压病提供了参考依据.

Objective To investigate the causal relationship between type 2 diabetes mellitus(T2DM)and hypertension using Mendelian randomization(MR),and to predict potential Chinese herbal medicines(CHMs)that may regulate both T2DM and hypertension.Methods Summary data from genome-wide association studies(GWAS)on T2DM and hypertension were obtained from public databases.MR analysis was performed using the TwoSampleMR package in R,with the inverse variance weighted(IVW)method as the primary approach for assessing genetic causal effects.Functional enrichment analysis of instrumental variable-proximal genes was conducted to explore relevant signaling pathways.The Coremine database was used to predict CHMs with potential regulatory effects on these genes,and information on their properties,flavors,meridian tropisms,and efficacy was summarized.Results MR analysis revealed that T2DM was associated with an increased risk of hypertension.Instrumental variable-proximal genes were mainly enriched in signaling pathways such as phosphatidylinositol-3-hydroxykinase(PI3K)/protein kinase B(Akt),rat sarcoma(Ras),mitogen-activated protein kinase(MAPK),and endocrine resistance.CHMs were predominantly warm in property,supplemented by cold and neutral properties;bitter in flavor,complemented by sweet and pungent flavors;primarily associated with the liver meridian,followed by the lung,spleen,stomach,and kidney meridians.Their main efficacy was clearing heat,followed by tonifying deficiency,and also included activating blood circulation,regulating qi,dispelling wind-dampness,resolving phlegm and relieving cough and asthma,and promoting diuresis and draining dampness.Representative CHMs with these high-frequency characteristics included Salviae Miltiorrhizae Radix et Rhizoma(Danshen),Ginseng Radix et Rhizoma(Renshen),Coptidis Rhizoma(Huanglian),Scutellariae Radix(Huangqin),Curcumae Radix(Yujin),Curcumae Longae Rhizoma(Jianghuang),and Astragali Radix(Huangqi).Conclusion This MR analysis suggests that T2DM may increase the risk of hypertension,potentially mediated through signaling pathways such as PI3K/Akt,MAPK,and Ras.Potential regulatory CHMs mainly include heat-clearing herbs,tonifying herbs,phlegm-resolving and cough-relieving herbs,and qi-regulating herbs,providing a reference for the prevention and treatment of diabetes complicated with hypertension using traditional Chinese medicine.
