小视野高清DWI在鉴别胰腺良恶性肿瘤中的应用OA
Application of field-of-view optimized and constrained undistorted single-shot DWI in differentiation of benign and malignant pancreatic tumors
目的:探讨小视野高清DWI(FOCUS-DWI)在鉴别胰腺良恶性肿瘤中的应用价值.方法:前瞻性收集行MRI检查的胰腺肿瘤患者55例,其中良性23例,恶性32例.均行常规DWI和FOCUS-DWI扫描,测量并计算2种图像的SNR、CNR和ADC值,并对图像质量进行主观评分.2种扫描序列图像的SNR、CNR、ADC值及主观评分比较行配对样本t检验,同一成像方法良恶性肿瘤间的SNR、CNR及ADC值比较行独立样本t检验.结果:FOCUS-DWI图像在变形失真与伪影、解剖细节、病变明显度方面的评分,以及SNR和CNR均高于常规DWI图像,差异均有统计学意义(均P<0.05).良性肿瘤常规DWI图像及FOCUS-DWI图像ADC值均高于恶性肿瘤(均P<0.05),良性肿瘤常规DWI图像ADC值较FOCUS-DWI图像的低,恶性肿瘤常规DWI图像ADC值较FOCUS-DWI图像的高(均P<0.05).良性肿瘤常规DWI和FOCUS-DWI图像ADC值离散度分别为23.2%(0.64/2.76)和21.6%(0.65/3.01);恶性肿瘤常规DWI和FOCUS-DWI图像ADC值离散度分别为9.7%(0.15/1.55)和10.2%(0.14/1.37).结论:FOCUS-DWI能提高胰腺肿瘤的图像质量,具有良好的应用前景.
Objective:To explore the application value of field-of-view optimized and constrained undistorted single-shot DWI(FOCUS-DWI)in differentiating benign and malignant pancreatic tumors.Methods:A total of 55 pancreatic tumor patients who underwent MRI examination were prospectively collected,including 23 cases with benign tumors and 32 cases with malignant tumors.All patients underwent conventional DWI and FOCUS-DWI scanning,and the SNR,CNR and ADC values of the images were measured and analyzed,and the image quality was subjectively evaluated.The paired t test was used to analyze the differences in SNR,CNR,ADC values and subjective scores between conventional DWI images and FOCUS-DWI images,while the independent-sample t test was used to anlayze the differences in SNR,CNR and ADC values between benign and malignant tumors.Results:The FOCUS-DWI images had the higher scores of deformation distortion and artifacts,anatomical details and lesion conspicuity,and higher SNR,CNR values compared with the conventional DWI images,and all differences were statistically significant(all P<0.05).The ADC values in the conventional DWI and FOCUS-DWI images of benign tumors were higher than those of malignant tumors(both P<0.05).The ADC value of benign tumors in conventional DWI images was lower than that in FOCUS-DWI images,while the ADC value of malignant tumors in conventional DWI images was higher than that in FOCUS-DWI images(both P<0.05).The dispersion of ADC values of benign tumors in the conventional DWI and FOCUS-DWI images were 23.2%(0.64/2.76)and 21.6%(0.65/3.01),while those of malignant tumors were 9.7%(0.15/1.55)and 10.2%(0.14/1.37),respectively.Conclusion:FOCUS-DWI can improve the image quality of pancreatic tumors and demonstrates promising clinical potential.
