血管斑块特征及脂肪衰减指数对冠心病心肌缺血的预测价值OA
Predictive value of plaque characteristics and fat attenuation index for myocardial ischemia in coronary heart disease
目的:探讨血管斑块特征与冠状动脉周围脂肪衰减指数(FAI)联合对冠状动脉粥样硬化性心脏病(冠心病)心肌缺血的预测价值.方法:回顾性选择冠心病患者265例(398支血管),测量病变血管的斑块特征、三支主干血管近端FAI及CT血流储备分数(CT-FFR).依据CT-FFR分为心肌缺血组(CT-FFR≤0.8,91例)和无心肌缺血组(CT-FFR>0.8,174例).采用logistic回归分析评估冠心病心肌缺血的独立预测因子.绘制ROC曲线分析各独立预测因子对冠心病心肌缺血的预测效能.结果:2组糖尿病占比差异有统计学意义(P<0.05).心肌缺血组较无心肌缺血组病变长度更长、冠状动脉狭窄程度更重、正性重构更明显、斑块总体积及钙化体积增加、FAI升高、低密度斑块占比大(均P<0.05).单因素及多因素logistic回归分析发现,糖尿病、冠状动脉狭窄程度及FAI是冠心病心肌缺血的独立预测因子(均P<0.05).ROC曲线分析显示,糖尿病、冠状动脉狭窄程度及FAI诊断冠心病心肌缺血的AUC分别为0.564、0.812、0.708,联合诊断的AUC为0.850.结论:糖尿病、冠状动脉狭窄程度及FAI对冠心病心肌缺血有较好的预测价值.
Objective:To explore the predictive value of plaque characteristics and pericoronal fat attenuation index(FAI)in the diagnosis of myocardial ischemia in coronary heart disease.Methods:A retrospective analysis was performed on 265 patients with coronary heart disease(398 coronary arteries).And the plaque characteristics,the proximal FAI of the three main coronary arteries and CT fractional flow reserve(CT-FFR)were measured.The patients were divided into the myocardial ischemia group(91 cases,CT-FFR≤0.8)and non-myocardial ischemia group(174 cases,CT-FFR>0.8)according to CT-FFR values.Logistic regression analysis was used to evaluate the independent predictors of myocardial ischemia in coronary heart disease.The ROC curves were plotted to analyze the performance of the independent predictors on myocardial ischemia in coronary heart disease.Results:There was a statistically significant difference in the proportion of diabetes between the two groups(P<0.05).The myocardial ischemia group showed a longer lesion,more severe degree of coronary artery stenosis,more obvious degree of the positive remodeling,larger volume of plaque and calcification,a larger FAI,and a larger proportion of low-density plaques compared to the non-myocardial ischemia group(all P<0.05).Univariate and multivariate logistic regression analysis revealed that diabetes,degree of coronary artery stenosis and FAI were the independent predictors of myocardial ischemia in coronary heart disease(all P<0.05).ROC curve analysis showed that the AUC values for diagnosing myocardial ischemia in coronary heart disease by diabetes,degree of coronary artery stenosis and FAI were 0.564,0.812 and 0.708,respectively,and the AUC for the combined diagnosis was 0.850.Conclusion:Diabetes mellitus,stenosis degree and FAI have good predictive value in the diagnosis of myocardial ischemia in coronary heart disease.
