目的 探讨罗哌卡因局部浸润辅助全身麻醉(简称全麻)对脊柱手术患者术后疼痛与快速康复的影响.方法 选取医院 2023 年8 月至 2024 年 6 月收治的拟行脊柱手术的患者 100 例,采用随机抽签法分为对照组和观察组,各 50 例.两组患者均行全麻,对照组患者术毕即刻连接静脉自控镇痛泵(PCIA),予舒芬太尼镇痛;观察组患者术毕即刻在切口周围局部浸润盐酸罗哌卡因注射液,待切口缝合完毕,采用与对照组相同的PCIA方案行术后镇痛.结果 与对照组比较,观察组患者术后 4,8,12,24,48 h的运动状态与静息状态下切口疼痛程度均显著降低(P<0.05);术后 24,48 h的疼痛因子(神经肽Y、5-羟色胺、组胺、P物质)和炎性因子(肿瘤坏死因子-α、白细胞介素 1β、白细胞介素 6)水平均显著降低(P<0.05);术后引流管的拔除时间、首次排气时间、辅助下床时间、住院时间均显著缩短(P<0.05);术后 24,48 h的凝血功能(凝血酶原时间、活化部分凝血酶时间、纤维蛋白原)无显著差异(P>0.05).观察组患者术后的不良反应、并发症发生率分别为 4.00%和 2.00%,显著低于对照组的 18.00%和 14.00%(P<0.05).结论 罗哌卡因局部浸润辅助全麻,能显著减轻脊柱手术患者的术后疼痛与炎性反应,减少并发症,促进快速康复,且安全性良好.

Objective To investigate the effect of ropivacaine local infiltration-assisted general anesthesia on postoperative pain and enhanced recovery in patients undergoing spinal surgery.Methods A total of 100 patients admitted to the hospital from August 2023 to June 2024 and scheduled to undergo spinal surgery were selected and randomly divided into the control group and the observation group,with 50 cases in each group.The patients in the two groups underwent general anesthesia,the patients in the control group were immediately connected to a patient-controlled intravenous analgesia(PCIA)pump after surgery and received sufentanil for analgesia,while the patients in the observation group received local infiltration of Ropivacaine Hydrochloride Injection around the incision immediately after surgery,and received the same PCIA regimen as in the control group for postoperative analgesia after suturing incision.Results Compared with those in the control group,the degree of incision pain during both active and resting states in the observation group significantly reduced at 4,8,12,24,and 48 h after surgery(P<0.05),the levels of pain-related factors(neuropeptide Y,serotonin,histamine,substance P)and inflammatory factors(tumor necrosis factor-α,interleukin-1β,interleukin-6)in the observation group significantly decreased at 24 and 48 h after surgery(P<0.05),the time for drain removal,first flatus,and assisted ambulation,and the length of hospital stay in the observation group significantly shortened(P<0.05).There were no significant differences in coagulation function(prothrombin time,activated-partial thromboplastin time,fibrinogen)between the two groups at 24 and 48 h after surgery(P>0.05).The incidence rates of adverse reactions and complications in the observation group were 4.00%and 2.00%,which were significantly lower than 18.00%and 14.00%in the control group(P<0.05).Conclusion Ropivacaine local infiltration-assisted general anesthesia can significantly alleviate postoperative pain and inflammatory response in patients undergoing spinal surgery,reduce complications,promote rapid recovery,and has good safety.
