MR成像技术探讨原发性膝关节退行性变程度与半月板损伤程度的关系研究OA
Study on the Relationship Between the Severity of Primary Degenerative Changes of the Knee Joint and the Degree of Meniscal Injury Using MR Imaging Techniques
目的 探讨原发性膝关节退行性变程度与半月板损伤程度的相关性.方法 选取驻马店市中医院 2022 年 12月至 2024 年 12 月的 64 例膝骨关节炎(KOA)患者进行回顾性分析,均已完成磁共振(MR)检查,根据检查结果评估患者膝关节退行性变程度、半月板损伤程度,并进行相关性分析.结果 患者KOA病变程度Ⅰ级 19 例(29.69%)、Ⅱ级20例(31.25%)、Ⅲ级14例(21.88%)、Ⅳ级11例(17.19%);半月板损伤程度0级4例(6.25%)、Ⅰ级20例(31.25%)、Ⅱ级 22 例(34.38%)、Ⅲ级 18 例(28.13%).KOA病变程度Ⅰ级患者半月板损伤程度 0 级、Ⅰ级占比最高,Ⅱ级、Ⅲ级患者半月板损伤程度Ⅱ级占比更高,Ⅳ级患者半月板损伤程度Ⅲ级占比更高(P<0.05).半月板损伤Ⅲ级患者内侧与外侧胫骨平台、股骨内侧髁与外侧髁处的T2 弛豫时间高于其他半月板损伤程度患者,Ⅱ级患者上述关节软骨T2 弛豫时间高于 0 级与Ⅰ级,Ⅰ级患者上述关节软骨T2 弛豫时间高于 0 级(P<0.05).KOA病变程度与半月板损伤程度呈正相关(r=0.452,P<0.05).结论 根据MR检查结果,KOA病变程度高者半月板损伤高级别患者占比更多,而半月板损伤程度高者关节软骨T2 弛豫时间更长,膝关节退行性变程度与半月板损伤程度呈正相关.
Objective To investigate the correlation between the severity of primary degenerative changes of the knee joint and the degree of meniscal injury.Methods A retrospective analysis was conducted on 64 patients with knee osteoarthritis(KOA)treated at Zhumadian Hospital of Traditional Chinese Medicine from December 2022 to December 2024.All patients had completed magnetic resonance(MR)examinations.Based on MR findings,the severity of knee joint degenerative changes and the degree of meniscal injury were assessed,and correlation analysis was performed.Results The severity of KOA lesions was grade Ⅰ in 19 cases(29.69%),grade Ⅱ in 20 cases(31.25%),grade Ⅲ in 14 cases(21.88%),and grade Ⅳ in 11 cases(17.19%).The degree of meniscal injury was grade 0 in 4 cases(6.25%),grade Ⅰ in 20 cases(31.25%),grade Ⅱ in 22 cases(34.38%),and grade Ⅲ in 18 cases(28.13%).In patients with KOA grade Ⅰ,the proportions of meniscal injury grade 0 and grade Ⅰ were the highest;in patients with KOA grade Ⅱ and grade Ⅲ,meniscal injury grade Ⅱ accounted for a higher proportion;in patients with KOA grade Ⅳ,meniscal injury grade Ⅲ accounted for a higher proportion(P<0.05).Patients with meniscal injury grade Ⅲ had higher T2 relaxation times at the medial and lateral tibial plateaus and the medial and lateral femoral condyles than those with other degrees of meniscal injury;patients with grade Ⅱ meniscal injury had higher T2 relaxation times in the above articular cartilage than those with grade 0 and grade Ⅰ;patients with grade Ⅰ meniscal injury had higher T2 relaxation times in the above articular cartilage than those with grade 0(P<0.05).The severity of KOA lesions was positively correlated with the degree of meniscal injury(r=0.452,P<0.05).Conclusion Based on MR findings,patients with more severe KOA had a higher proportion of high-grade meniscal injury,and patients with more severe meniscal injury had longer T2 relaxation times of articular cartilage.The severity of knee joint degenerative changes is positively correlated with the degree of meniscal injury.
