目的:基于嵌套阵列芯片构建患者来源胃癌类器官(PDO)模型,探讨其在个性化药物筛查中的应用价值.方法:收集8例胃癌患者术中切除的组织标本,经酶解消化处理后与50%基质胶混合,将8 µL混合物播种到芯片的3D植入孔中完成PDO培养,通过苏木精-伊红染色和免疫组化染色,对胃癌PDO及原始肿瘤组织进行病理形态学及免疫分子标记同质性验证.通过对紫杉醇(paclitaxel)、5-氟尿嘧啶(5-FU)、奥沙利铂(oxaliplatin)单药以及5-FU+oxaliplatin(FOLFOX)联合化疗方案进行药物敏感性筛查,并结合影像学评估,验证芯片上胃癌PDO模型在药物筛查中的可行性.结果:8例标本中成功培养6例胃癌PDO,包括2例肠型,2例弥漫型和2例混合型.PDO的形态及生长特性存在个体差异,但其形态学特征及免疫分子表达均与对应的原发肿瘤高度一致.更重要的是,芯片上的药物反应再现了个体患者的异质性反应,其敏感性差异与临床观察到的疗效趋势相符.结论:本研究初步验证了基于嵌套阵列芯片的胃癌PDO模型作为体外药物筛选工具的可行性.

AIM:To construct a patient-derived gastric cancer organoid(PDO)model using a nested array chip and evaluate its utility in personalized drug screening.METHODS:Tissue specimens were obtained from eight gas-tric cancer patients during surgical resection.The specimens were digested,dissociated,and then mixed with 50%Matri-gel.An 8 µL aliquot of the mixture was seeded into the 3D implantation holes of the chip.Hematoxylin and eosin(HE)staining,along with immunohistochemical staining,were employed to validate the histomorphological and immunological marker consistency between the PDOs and the primary tumor tissues.Drug sensitivity screening was conducted using monotherapies of Paclitaxel,5-fluorouracil(5-FU),and oxaliplatin,as well as the 5-FU+oxaliplatin(FOLFOX)combina-tion regimen.The viability of the chip-based PDO model for drug screening was evaluated by integrating imaging analyses.RESULTS:Six PDO lines were successfully cultured,encompassing 2 intestinal-type,2 diffuse-type,and 2 mixed-type organoids.The PDOs displayed individual variations in morphology and growth characteristics.Notably,they exhibited identical morphological features and comparable immunological marker expression patterns to their corresponding primary tumors.More significantly,the drug response outcomes obtained from the chip-based model mirrored the heterogeneous re-sponses among individual patients,with the differential drug sensitivity profiles aligning well with the actual clinical re-sponses.CONCLUSION:The preliminary verification established the feasibility of utilizing PDOs on this nested array chip as an in vitro platform for drug screening.
