肛周坏死性筋膜炎术后预后不良危险因素相关分析OA

Objective To explore the independent risk factors for poor prognosis after surgery for perianal necrotizing fasciitis(PNF).Methods A total of 98 patients with PNF admitted from December 2013 to December 2023 were retrospectively enrolled,and randomly divided into a favorable prognosis group(80 cases)and a poor prognosis group(18 cases).General data,laboratory indicators and comorbidities of the two groups were collected for univariate analysis and multivariate Logistic regression analysis.Results The inci‐dence of poor prognosis after surgery for PNF was 18.4%(18/98).Univariate analysis showed that there were significant differences in disease course,levels of C-reactive protein(CRP),white blood cell count(WBC)and blood glucose between the two groups(P<0.05).Multivariate Logistic regression analysis indicated that disease course(B=0.156,OR=1.169,95%CI=1.002‐1.364),CRP(B=0.091,OR=1.095,95%CI=1.006‐1.191),WBC(B=0.29,OR=1.337,95%CI=1.111‐1.61)and blood glucose(B=0.688,OR=1.99,95%CI=1.377‐2.876)were independent risk factors for poor prognosis after surgery for PNF(P<0.05).Conclusion Prolonged disease course,elevated admission levels of CRP,WBC and blood glucose are independent risk factors for poor prognosis after surgery for PNF(P<0.05).Clinical practice should focus on monitoring the above indicators for early intervention.
