多种健康指标与高尿酸血症的相关性:基于飞行人员与普通人群的横断面分析OA
Correlation between multiple health indicators and hyperuricemia:a cross-sectional analysis of aircrew and general population
目的 通过飞行人员与普通人群的横断面研究,分析高尿酸血症(HUA)以及尿酸变化的影响因素.方法 整群抽取 2024 年某部队 412 例体检人员作为研究对象,收集其生活方式、健康情况等相关资料,对身体健康指标开展相关性分析,并进一步采用多变量模型分析HUA以及尿酸的影响因素.结果 412 例体检人员HUA总体患病率为31.8%,其中飞行人员患病率为 22.7%,普通人群患病率为 32.3%.对全部 412 名体检人员的年龄、睡眠时间、体质量指数(BMI)、肝功能指标进行相关性分析显示:BMI与舒张压(DBP)、收缩压(SBP)、丙氨酸氨基转移酶(ALT)和γ谷氨酰氨基转移酶(GGT)呈正相关(P<0.05),与总胆红素(TB)和直接胆红素(DB)呈负相关(P<0.05).390 名普通人群的分析显示:BMI 与 DBP、ALT、GGT 呈正相关(P<0.05),与TB、DB呈负相关(P<0.05).22 名飞行人员的相关性分析则显示:年龄与 ALT、碱性磷酸酶(ALP)呈正相关(P<0.05),与白蛋白(ALB)呈负相关(P<0.05);BMI与DBP、ALT、ALP 和GGT呈正相关(P<0.05),与TB、DB呈负相关(P<0.01);尿酸与GGT呈正相关(P<0.01).logistic回归分析HUA与人体健康指标之间的关联,计算并展示了OR(95%CI):BMI(P<0.01)、ALT(P<0.01)、天冬氨酸氨基转移酶(AST)(P<0.01)、GGT(P<0.01)和总蛋白(TP)(P<0.05)与 HUA 发生概率呈显著正相关[BMI 1.23(1.12,1.34);ALT 1.03(1.02,1.04);AST 1.04(1.02,1.07);GGT 1.02(1.01,1.04);TP 1.07(1.01,1.13)].线性回归分析人体健康指标(四分位数)与尿酸含量的关联,计算并展示了 β(95%CI):BMI(P<0.01)、AST(P<0.01)、ALT(P<0.01)、ALP(P<0.05)和DB(P<0.05)在第四分位与尿酸含量呈显著相关[BMI Q4 为0.16(0.06,0.26),AST Q4 为0.15(0.06,0.25),ALT Q4 为 0.13(0.04,0.22),ALP Q4为0.13(0.03,0.22),DB Q4 为-0.11(-0.21,-0.01)];GGT在第三分位(P<0.01)和第四分位(P<0.01)与尿酸含量呈显著正相关[GGT Q3 为0.14(0.05,0.23),Q4 为0.22(0.12,0.31)].结论 BMI及肝功能指标异常对尿酸的影响较大,且高值DB可能是HUA的保护因素.
Objective To analyze the influencing factors of hyperuricemia(HUA)and changes in uric acid through a cross-sectional study of aircrew and the general population.Methods In 2024,a total of 412 individuals undergoing health examinations in a military unit were selected as research subjects by cluster sampling.Data on lifestyle,health status,and related indicators were collected.Correlation analysis was performed on physical health indicators,and further multiple-exposure models were used to analyze the influencing factors of HUA and uric acid levels.Results The overall prevalence of HUA among the 412 examinees was 31.8%,with a prevalence of 22.7%in aircrew and 32.3%in the general population.Correlation analysis of age,sleep duration,body mass index(BMI),and liver function indicators among all 412 participants showed that BMI was positively correlated with diastolic blood pressure(DBP),systolic blood pressure(SBP),alanine aminotransferase(ALT),and gamma-glutamyl transferase(GGT)(P<0.05),and negatively correlated with total bilirubin(TB)and direct bilirubin(DB)(P<0.05).Among the 390 individuals in the general population,BMI was positively correlated with DBP,ALT,and GGT(P<0.05),and negatively correlated with TB and DB(P<0.05).Among the 22 aircrew,correlation analysis revealed that age was positively correlated with ALT and alkaline phosphatase(ALP)(P<0.05),and negatively correlated with albumin(ALB)(P<0.05).BMI was positively correlated with DBP,ALT,ALP,and GGT(P<0.05),and negatively correlated with TB and DB(P<0.01).Uric acid was positively correlated with GGT(P<0.01).Logistic regression analysis was used to examine the association between HUA and human health indicators,and the OR(95%CI)was calculated and presented:BMI(P<0.01),ALT(P<0.01),aspartate aminotransferase(AST)(P<0.01),GGT(P<0.01),and total protein(TP)(P<0.05)were significantly positively correlated with the probability of HUA occurrence[BMI:1.23(1.12,1.34),ALT:1.03(1.02,1.04),AST:1.04(1.02,1.07),GGT:1.02(1.01,1.04),TP:1.07(1.01,1.13)].Linear regression analysis was used to examine the association between human health indicators(quartiles)and uric acid levels,and the β(95%CI)was calculated and presented:BMI(P<0.01),AST(P<0.01),ALT(P<0.01),ALP(P<0.05),and DB(P<0.05)in the fourth quartile were significantly correlated with uric acid levels[BMI Q4:0.16(0.06,0.26),AST Q4:0.15(0.06,0.25),ALT Q4:0.13(0.04,0.22),ALP Q4:0.13(0.03,0.22),DB Q4:-0.11(-0.21,-0.01)].GGT was significantly positively correlated with uric acid levels in the third quartile(P<0.01)and the fourth quartile(P<0.01)[GGT Q3:0.14(0.05,0.23),Q4:0.22(0.12,0.31)].Conclusion BMI and abnormal liver function indicators have a significant impact on uric acid levels,and a higher DB level may be a protective factor against HUA.
