成纤维细胞生长因子21及闭锁小带蛋白1水平与急性冠脉综合征患者冠状动脉斑块稳定性、狭窄程度的关系OA
Relationship between Fibroblast Growth Factor 21 and Zonula Occludens Protein-1 and the Plaque Stability and Stenosis Degree of Coronary Artery in Patients with Acute Coronary Syndrome
目的 分析成纤维细胞生长因子21(FGF21)、闭锁小带蛋白1(ZO-1)水平与急性冠脉综合征(ACS)患者冠状动脉斑块稳定性、狭窄程度的关系.方法 回顾性选取2022年3月—2025年3月中国人民解放军联勤保障部队第九六〇医院收治的ACS患者215例为研究对象.收集患者一般资料、FGF21水平、ZO-1水平、冠状动脉斑块稳定性、冠状动脉狭窄程度.采用多因素Logistic回归分析探讨ACS患者冠状动脉斑块稳定性、狭窄程度的影响因素;采用ROC曲线分析FGF21、ZO-1水平及其联合对ACS患者冠状动脉斑块不稳定、重度狭窄的预测价值.结果 215例ACS患者中,冠状动脉斑块稳定60例,不稳定155例;冠状动脉轻中度狭窄116例,重度狭窄99例.多因素Logistic回归分析结果显示:糖尿病、高脂血症、TG、HDL-C、心肌肌钙蛋白T(cTnT)、FGF21水平、ZO-1水平为ACS患者冠状动脉斑块不稳定的影响因素(P<0.05);ACS家族史、TG、HDL-C、cTnT、FGF21水平、ZO-1水平为ACS患者冠状动脉重度狭窄的影响因素(P<0.05).ROC曲线分析结果显示:FGF21、ZO-1水平及二者联合预测ACS患者冠状动脉斑块不稳定的AUC分别为0.829[95%CI(0.768~0.891)]、0.755[95%CI(0.682~0.828)]、0.871[95%CI(0.814~0.929)];FGF21、ZO-1水平及二者联合预测ACS患者冠状动脉重度狭窄的AUC分别为0.802[95%CI(0.743~0.860)]、0.794[95%CI(0.735~0.853)]、0.874[95%CI(0.828~0.921)].结论 FGF21、ZO-1水平升高为ACS患者冠状动脉斑块不稳定、重度狭窄的保护因素,其均对ACS患者冠状动脉斑块不稳定、重度狭窄有一定预测价值,且二者联合的预测价值较高.
Objective To analyze the relationship between fibroblast growth factor 21(FGF21)and zonula occludens protein-1(ZO-1)and the plaque stability and stenosis degree of coronary artery in patients with acute coronary syndrome(ACS).Methods A retrospective analysis was conducted on 215 patients with ACS admitted to the 960th Hospital of the Joint Service Support Force of the Chinese People's Liberation Army from March 2022 to March 2025.General data,levels of FGF21 and ZO-1,plaque stability of coronary artery,and the stenosis degree of coronary artery were collected.Multivariate Logistic regression analysis was used to explore the influencing factors for the plaque stability and stenosis degree of coronary artery in patients with ACS.The ROC curve was employed to evaluate the predictive value of FGF21 and ZO-1 levels and their combination for unstable coronary artery plaque and severe stenosis of coronary artery in patients with ACS.Results Among the 215 ACS patients,60 had stable coronary plaques and 155 had unstable coronary plaques;116 had mild-to-moderate coronary plaque stenosis and 99 had severe coronary plaque stenosis.Multivariate Logistic regression analysis showed that diabetes,hyperlipidemia,TG,HDL-C,cardiac troponin T(cTnT),FGF21 level,and ZO-1 level were influencing factors for unstable coronary artery plaque in patients with ACS(P<0.05);family history of ACS,TG,HDL-C,cTnT,FGF21 level,and ZO-1 level were influencing factors for severe stenosis of coronary artery in patients with ACS(P<0.05).ROC curve analysis results showed that the AUC of FGF21 and ZO-1 levels and their combination in predicting unstable coronary artery plaque in patients with ACS was 0.829[95%CI(0.768-0.891)],0.755[95%CI(0.682-0.828)],0.871[95%CI(0.814-0.929)],respectively;the AUC of FGF21 and ZO-1 levels and their combination in predicting severe stenosis of coronary artery in patients with ACS was 0.802[95%CI(0.743-0.860)],0.794[95%CI(0.735-0.853)],0.874[95%CI(0.828-0.921)],respectively.Conclusion Elevated levels of FGF21 and ZO-1 are protective factors for unstable coronary artery plaque and severe stenosis of coronary artery in patients with ACS.Both of the two have certain predictive value for unstable coronary artery plaque and severe stenosis of coronary artery in patients with ACS,and their combination has higher predictive value.
