首发精神分裂症病人基于糖代谢紊乱的自杀意念风险预测模型的构建与验证OA
Construction and validation of a risk prediction model for suicidal ideation in first-episode schizophrenia patients based on glucose metabolism disorders
目的:为首发未用药的精神分裂症病人开发基于糖代谢紊乱的自杀意念风险预测模型.方法:纳入333例精神分裂症病人,以7∶3的比例随机分为训练组(n=233)和验证组(n=100).通过阳性与阴性症状量表(PANSS)评估精神病性症状,并基于其五因子模型评估抑郁和认知因子.同时收集空腹血糖、糖化血红蛋白、空腹胰岛素,并计算胰岛素抵抗指数.使用贝克自杀意念量表将病人分为有自杀意念和无自杀意念.训练组中,通过LASSO和二元Logistic回归分析识别相关变量,并构建列线图预测自杀意念风险.使用受试者工作特征(ROC)曲线下面积(AUC)评估模型预测能力,校准曲线评估准确性,决策曲线分析(DCA)评估临床实用性.结果:基于阳性症状得分、抑郁因子得分、空腹血糖和空腹胰岛素构建了列线图.训练组和验证组的AUC分别为0.776[95%CI(0.715,0.837)]和0.763[95%CI(0.665,0.860)],表明模型具有良好的判别能力,校准分析和DCA证实了列线图的临床适用性.结论:构建的预测精神分裂症病人自杀意念风险列线图,有助于临床上识别高自杀风险病人,为及时实施预防策略,降低自杀风险提供参考.
Objective:To develop a risk prediction model for suicidal ideation in first-episode schizophrenia patients based on glucose metabolism disorders.Methods:A total of 333 patients with schizophrenia were randomly divided into a training group(n=233)and a validation group(n=100)in a 7∶3 ratio.Positive and Negative Syndrome Scale(PANSS)were used to evaluate psychiatric symptoms.Depressive and cognitive factors were evaluated based on the five-factor model.Fasting blood glucose,glycated hemoglobin,and fasting insulin were collected.The insulin resistance index was calculated.Patients were categorized into groups with and without suicidal ideation by using the Beck Suicide Ideation Scale.In the training group,relevant variables were identified through LASSO and Logistic regression analysis.A Nomogram was constructed to predict the risk of suicidal ideation.The area under the ROC curve(AUC)was used to evaluate the predictive ability of the model.Calibration curve was used to evaluate accuracy.Decision curve analysis(DCA)was used to evaluate clinical practicality.Results:A Nomogram was constructed based on positive symptoms scores,depressive factors scores,fasting blood glucose,and fasting insulin.The AUC of the training group and validation group were 0.776(95%CI 0.715-0.837)and 0.763(95%CI 0.665-0.860),respectively,indicating good discriminative ability of the model.Calibration analysis and DCA confirmed the clinical applicability of the nomogram.Conclusions:The Nomogram constructed in this study for predicting the risk of suicidal ideation in patients with schizophrenia could help identify high-risk suicide patients in clinical practice,provide reference for timely implementation of prevention strategies,and reduce suicide risk.
