从"瘀热"探讨糖尿病合并脂代谢异常的病机与论治OA
Exploring the Pathogenesis and Treatment of Diabetic Dyslipidemia from the Perspective of the"Stasis-Heat"Theory
糖尿病合并脂代谢异常是临床常见、危害显著的内分泌代谢紊乱状态,显著增加心脑血管事件风险.文章以"瘀热"理论为基础,对糖尿病合并脂代谢异常的病机演变进行阐述,其核心病机为本虚标实,以中焦郁热、气阴两虚为本虚之基,热灼津血、炼液成瘀为标实之要.因"瘀""热"相互胶结,形成"瘀热互结、蕴毒损络"的动态演变关系链,促使疾病向血管并发症进展.基于病程不同阶段特点,分阶段采用清透郁热、化瘀消脂、解毒通络治法,旨在实现调控糖脂代谢与防治血管并发症的协同效应,为中医辨治提供新思路.
Diabetic dyslipidemia is a clinically common and significantly harmful endocrine-metabolic disorder that marked-ly increases the risk of cardiovascular and cerebrovascular events.Based on the theory of"Stasis-Heat",this article elucidates the pathogenic evolution of diabetic dyslipidemia.Its core pathogenesis lies in deficiency in origin and excess in manifestation.De-ficiency in origin is rooted in stagnant heat in the middle energizer and deficiency of both qi and yin,while excess in manifestation primarily involves heat scorching body fluids and blood,congealing fluids into stasis.Due to the mutual entanglement of"stasis"and"heat",a dynamic evolutionary chain characterized by"stasis-heat interaction,toxin accumulation,and collateral damage"is formed,driving the progression of the disease towards vascular complications.Based on the characteristics of different disease stages,treatment is stratified using methods to clear and disperse stagnant heat,resolve stasis and dissipate lipids,and detoxify and unblock collaterals.This approach aims to achieve the synergistic effect of regulating glucose-lipid metabolism and preven-ting vascular complications,providing a new perspective for TCM pattern differentiation and treatment.
