目的:探讨精神科护士的人文执业能力、道德困境、道德复原力类别,分析不同类别与离职意愿的关系.方法:于2025年1月20日—2月18日选取广东省1所精神专科医院及3所二级甲等及以上综合医院的370名精神科护士为研究对象,采用一般资料调查表、护士人文执业能力测评问卷、精神科护士道德困境量表、中文版拉什顿道德复原力量表及辞职意向量表进行调查.采用Mplus 8.3进行潜在剖面分析,分析精神科护士的人文执业能力、道德困境、道德复原力水平的不同类别,采用回归分析探讨不同类别护士的离职意愿差异.结果:基于370名精神科护士的人文执业能力、道德困境、道德复原力水平可将其分为3个类别,其中高能力-低困境组精神科护士占67.57%,平衡组精神科护士占21.89%,低能力-高困境组精神科护士占10.54%.不同类别的精神科护士离职意愿得分比较差异有统计学意义(F=14.587,P<0.01).回归分析结果显示,平衡组精神科护士和低能力-高困境组精神科护士离职意愿相对较高(P<0.05).结论:基于精神科护士的人文执业能力、道德困境、道德复原力水平可将其分为3个类别,低能力-高困境组精神科护士和平衡组精神科护士的离职意愿较高,管理者应给予更多的关注,制定针对性策略,以降低精神科护士的离职率.

Objective:To explore the categories based on humanistic practice competence,moral distress,and moral resilience of psychiatric nurses,and to analyze the relationship between different categories and turnover intention.Methods:A total of 370 psychiatric nurses from one psychiatric specialty hospital and three second grade A and above general hospitals in Guangdong Province were selected as the study subjects spanned from January 20,2025 to February 18,2025.A general information questionnaire,the Nurse Humanistic Practice Competence Scale,the Moral Distress Scale for psychiatric nurses,the Chinese version of Rushton Moral Resilience Scale,and Intent to Quit Scale were used for the survey.Mplus 8.3 was used for latent profile analysis to explore the different categories based on humanistic practice competence,moral distress,and moral resilience of psychiatric nurses.Regression analysis was used to explore the differences in turnover intention among nurses in different categories.Results:Psychiatric nurses could be divided into three categories based on their humanistic practice competence,moral distress,and moral resilience.The high competence-low distress group accounted for 67.57%.The balanced group accounted for 21.89%.The low competence-high distress group accounted for 10.54%.There were statistically significant differences in terms of turnover intention scores among psychiatric nurses in different categories(F=14.587,P<0.01).The results of the regression analysis showed that the turnover intention of psychiatric nurses in the balanced group and the low competence-high distress group were higher(P<0.05).Conclusions:Psychiatric nurses can be divided into three categories based on their humanistic practice competence,moral distress,and moral resilience.The turnover intention of psychiatric nurses in the low competence-high distress group and the balanced group are higher.Managers should pay more attention to these groups and develop targeted strategies to reduce the turnover rate of psychiatric nurses.
