Meta-analysis of the Effects of Exercise Intervention on Carotid Intima Media Thickness in Patients with Type 2 Diabetes Mellitus

目的 评价不同形式及强度的运动干预对2型糖尿病(T2DM)患者颈动脉内膜中层厚度(CIMT)的影响.方法 检索PubMed、Cochrane Library、Web of Science、中国知网、万方数据库、维普数据库及中国生物医学文献数据库中关于运动干预对T2DM患者CIMT影响的随机对照试验,检索时限为建库至2024年1月10日.2名研究者独立进行文献筛选、质量评价和数据提取,使用RevMan 5.4和Stata 16软件进行Meta分析,通过亚组分析和敏感性分析寻找异质性来源.结果 共纳入7篇文献,涉及680例受试者.Meta分析结果显示,接受运动干预的T2DM患者较对照组CIMT显著降低[MD=-0.06,95%CI(-0.09,-0.04),P<0.05].亚组分析结果显示,有氧运动组[MD=-0.10,95%CI(-0.12,-0.09)]和有氧运动联合抗阻运动组[MD=-0.06,95%CI(-0.08,-0.03)]T2DM患者的CIMT均明显低于对照组(均P<0.05),且有氧运动在CIMT降低方面的效应更显著(χ2=11.02,P<0.05);干预周期≤12周组[MD=-0.10,95%CI(-0.12,-0.07)]和12个月组[MD=-0.04,95%CI(-0.08,-0.01)]T2DM患者的CIMT均明显低于对照组(均P<0.05),且≤12周的干预周期在CIMT降低方面的效应更显著(χ2=5.95,P<0.05).Egger's检验未表现出显著的发表偏倚(t=0.58,P>0.05).结论 运动干预可使T2DM患者CIMT显著减低,其中有氧运动尤其是中等强度持续运动在改善CIMT方面的效果更优.

Objective To evaluate the effects of different forms and intensities of exercise intervention on carotid intima-media thickness(CIMT)in patients with type 2 diabetes mellitus(T2DM).Methods Randomized controlled trials on the effects of exercise intervention on CIMT in T2DM patients were searched in PubMed,Cochrane Library,Web of Science,CNKI,Wanfang Data,VIP and CBMdisc from their establishment to January 10,2024.Two investigators independently performed literature screening,quality evaluation and data extraction.The meta-analysis was conducted using RevMan 5.4 and Stata 16.The sources of heterogeneity were identified through subgroup analysis and sensitivity analysis.Results A total of 7 articles were included,involving 680 subjects.Meta-analysis revealed that CIMT was significantly reduced in T2DM patients receiving exercise intervention compared to the control group[MD=-0.06,95%CI(-0.09,-0.04),P<0.05].Subgroup analysis revealed that CIMT was significantly reduced in both the aerobic exercise group[MD=-0.10,95%CI(-0.12,-0.09)]and the aerobic+resistance exercise group[MD=-0.06,95%CI(-0.08,-0.03)]compared to the control group(P<0.05 for both),with a more significant effect on CIMT reduction in the aerobic exercise group(χ2=11.02,P<0.05);additionally,CIMT was significantly reduced in both the≤12-week intervention group[MD=-0.10,95%CI(-0.12,-0.07)]and the 12-month intervention group[MD=-0.04,95%CI(-0.08,-0.01)]compared to the control group(P<0.05 for both),with a more significant effect on CIMT reduction in the≤12-week intervention group(χ2=5.95,P<0.05).Egger's test showed no significant publication bias(t=0.58,P>0.05).Conclusion Exercise intervention significantly reduces CIMT in T2DM patients,with a superior effect on CIMT reduction in aerobic exercise,especially moderate-intensity continuous exercise.
