针刺结合十字灸治疗脑卒中后神经源性膀胱的疗效观察OA
Curative Effect of Acupuncture Combined with Cross Moxibustion in Treatment of Neurogenic Bladder After Stroke
目的 分析脑卒中后神经源性膀胱(PSNB)应用针刺结合十字灸治疗的疗效.方法 选取PSNB患者76例(均源于青岛市中医院)进行研究,选例时间:2022年3月—2023年9月,将其分为对照组(38例,常规治疗+针刺治疗)和研究组(38例,常规治疗+针刺结合十字灸治疗).分组方法参考随机数字表法分配原则.两组均治疗2个疗程,并随访1个月.比较两组临床指标:治疗后临床疗效,治疗前后排尿日记、尿动力学指标、国际前列腺症状评分(I-PSS)、美国国立卫生研究院卒中量表(NIHSS)、国际尿失禁咨询委员会尿失禁问卷表简表(ICI-Q-SF)、改良Barthel指数(MBI)评分.结果 总有效率组间比较:研究组94.74％(36/38)较高[治疗后,对照组76.32％(29/38)],P＜0.05.两组日均排尿次数、日均失禁次数、I-PSS、ICI-Q-SF、NIHSS评分、膀胱残余尿量均降低(治疗后,与治疗前比),且研究组较低(治疗后,相比于对照组);治疗后两组日均单次尿量、MBI评分、膀胱最大容量、最大尿流率、平均尿流率均升高(治疗后,与治疗前比),且研究组较高(治疗后,相比于对照组),P＜0.05.结论 PSNB患者应用针刺结合十字灸治疗效果明显,且有助于减轻患者临床症状,优化尿动力学指标,改善排尿功能、神经功能及日常活动能力,获益显著.
Objective To analyze the efficacy of acupuncture combined with cross moxibustion in the treatment of post-stroke neurogenic bladder(PSNB).Methods Seventy-six patients with PSNB(all from Qingdao Hospital of Traditional Chinese Medi-cine)were selected for the study.The selection time was from March 2022 to September 2023.They were divided into the control group(38 cases,routine treatment+acupuncture treatment)and the research group(38 cases,routine treatment+acupuncture combined with cross moxibustion treatment).The grouping method refers to the distribution principle of random number table method.Both groups were treated for 2 courses and followed up for 1 month.The clinical indicators of the two groups were com-pared,including the clinical efficacy after treatment,urination diary,urodynamic indicators,international prostate symptom score(I-PSS),national institutes of health stroke scale(NIHSS),international consultation on incontinence questionnaire-short form(ICI-Q-SF)and modified barthel index(MBI)scores before and after treatment.Results The total effective rate of the re-search group[94.74％(36/38)]was higher than that[76.32％(29/38)]of the control group(P＜0.05).The average daily u-rination frequency,the average daily incontinence frequency,I-PSS,ICI-Q-SF,NIHSS score and bladder residual urine vol-ume of the two groups decreased after treatment compared with those before treatment,and these indexes of the research group were lower compared with those of the control group.The average daily single urine volume,MBI score,maximum bladder capaci-ty,maximum urinary flow rate and average urinary flow rate of the two groups increased after treatment,compared with those before treatment,and the indexes of the research group were higher after treatment compared with those of the control group(P＜0.05).Conclusion Acupuncture combined with cross moxibustion is effective in the treatment of PSNB patients,and it is helpful to alle-viate the clinical symptoms,optimize the urodynamic indexes,improve the urination function,nerve function and daily activity abil-ity,obtaining the benefit significantly.
