目的 探讨急性缺血性脑卒中(AIS)患者血清成纤维细胞生长因子(FGF)20、细胞间黏附分子(ICAM)3水平及与神经功能缺损程度的关系.方法 前瞻性选取2022年1月至2024年8月解放军总医院第二医学中心收治的AIS患者156例作为AIS组,并通过美国国立卫生研究院卒中量表(NIHSS)评分将其分为轻度组、中度组和重度组;另选取同期在解放军总医院第二医学中心体检的健康志愿者78例作为对照组.收集AIS患者基线资料;采用酶联免疫吸附试验检测各组血清FGF20、ICAM3水平.采用Spearman相关分析AIS患者血清FGF20、ICAM3水平与神经功能缺损程度的相关性.采用有序多分类Logistic回归分析AIS患者神经功能缺损程度的影响因素.绘制受试者工作特征(ROC)曲线评估血清FGF20、ICAM3对AIS患者中、重度神经功能缺损和重度神经功能缺损的诊断价值.结果 与对照组相比,AIS组血清FGF20水平降低,血清ICAM3水平升高,差异均有统计学意义(P<0.05).NIHSS评分结果显示,156例AIS患者中,重度组47例、中度组55例、轻度组54例.中度组、重度组年龄、脑梗死体积大于轻度组,血清ICAM3水平高于轻度组,血清FGF20水平低于轻度组,差异均有统计学意义(P<0.05);重度组年龄、脑梗死体积大于中度组,血清ICAM3水平高于中度组,血清FGF20水平低于中度组,差异均有统计学意义(P<0.05).Spearman相关分析结果显示,AIS患者神经功能缺损程度与血清FGF20水平呈负相关(rs=-0.759,P<0.001),与血清ICAM3水平呈正相关(rs=0.747,P<0.001).有序多分类Logistic回归分析结果显示,血清ICAM3水平升高是AIS患者神经功能缺损程度加重的独立危险因素(P<0.05),血清FGF20水平升高是AIS患者神经功能缺损程度加重的独立保护因素(P<0.05).ROC曲线分析结果显示,血清FGF20、ICAM3联合预测AIS患者中、重度神经功能缺损的AUC为0.877,大于血清FGF20、ICAM3单独预测的0.811、0.803(Z=2.675,P=0.008;Z=2.572,P=0.010);血清FGF20、ICAM3联合预测AIS患者重度神经功能缺损的AUC为0.923,大于血清FGF20、ICAM3单独预测的0.830、0.824(Z=3.209,P=0.001;Z=3.457,P=0.001).结论 血清FGF20水平降低、血清ICAM3水平升高均与AIS患者神经功能缺损加重密切相关,且二者联合检测对AIS患者神经功能缺损加重具有良好的预测价值.

Objective To investigate the relationship between serum fibroblast growth factor(FGF)20,in-tercellular adhesion molecule(ICAM)3 levels and neurological deficits in patients with acute ischemic stroke(AIS).Methods A total of 156 AIS patients admitted to the Second Medical Center of Chinese PLA General Hospital from January 2022 to August 2024 were prospectively selected as AIS group,which were divided into mild group,moderate group and severe group by National Institutes of Health Stroke Scale(NIHSS)score.Another 78 healthy volunteers who came to the the Second Medical Center of Chinese PLA General Hospital for physical examination during the same period were selected as the control group.Baseline data of AIS pa-tients were collected.The levels of serum FGF20 and ICAM3 in each group were detected by enzyme-linked immunosorbent assay.Spearman correlation analysis was used to analyze the correlation between serum FGF20 and ICAM3 levels and the degree of neurological deficits in AIS patients.The influencing factors of neurological deficits in AIS patients were analyzed by ordinal multivariate Logistic regression analysis.The re-ceiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of serum FGF20 and ICAM3 for moderate to severe neurological deficits and severe neurological deficits in AIS patients.Results Compared with the control group,the serum FGF20 level in the AIS group decreased,and the ICAM3 level in-creased,with statistically significant differences(P<0.05).NIHSS score results showed that among the 156 AIS patients,there were 47 cases in the severe group,55 cases in the moderate group and 54 cases in the mild group.The age and cerebral infarction volume in the moderate group and the severe group were larger than those in the mild group,the serum ICAM3 level was higher than that in the mild group,and the serum FGF20 level was lower than that in the mild group,the differences were statistically significant(P<0.05).The age and cerebral infarction volume of the severe group were larger than those in the moderate group,the serum ICAM3 level was higher than that in the moderate group,and the serum FGF20 level was lower than that in the moderate group,the differences were statistically significant(P<0.05).Spearman correlation analysis showed that the degree of neurological deficit in AIS patients was negatively correlated with serum FGF20 level(rs=-0.759,P<0.001)and positively correlated with ICAM3 level(rs=0.747,P<0.001).Ordinal multivariate Logistic regression analysis showed that elevated serum ICAM3 level was an independent risk factor for the aggravation of neurological deficit in AIS patients(P<0.05),and elevated serum FGF20 level was an independent protective factor for the aggravation of neurological deficit in AIS patients(P<0.05).ROC curve analysis showed that the AUC of combined serum FGF20 and ICAM3 in predicting moderate to severe neurological deficits in AIS patients was 0.877,which was greater than 0.811 and 0.803 predicted by serum FGF20 and ICAM3 alone(Z=2.675,P=0.008;Z=2.572,P=0.010);the AUC of combined serum FGF20 and ICAM3 in predicting severe neurological deficits in AIS patients was 0.923,which was greater than 0.830 and 0.824 predicted by serum FGF20 and ICAM3 alone(Z=3.209,P=0.001;Z=3.457,P=0.001).Conclusion The decrease of serum FGF20 level and the increase of serum ICAM3 level are closely re-lated to the aggravation of neurological deficit in AIS patients,and the combined detection of the two indica-tors has a good predictive value for the aggravation of neurological deficit in AIS patients.
