目的 探讨冠心病(CHD)患者血清白细胞介素-5(IL-5)、谷胱甘肽过氧化物酶4(GPX4)水平与心功能的相关性.方法 选取2021年10月至2022年10月在北京市东城区第一人民医院就诊的CHD患者126例作为疾病组,其中美国纽约心脏病学会(NYHA)分级心功能Ⅱ级36例,Ⅲ级62例,Ⅳ级28例;另选取同期在北京市东城区第一人民医院体检健康者120例作为对照组.收集CHD患者的基线资料.采用酶联免疫吸附试验检测各组受试者血清IL-5、GPX4、脑钠肽(BNP)、肌酸激酶同工酶(CK-MB)水平.采用彩色多普勒超声诊断仪检测CHD患者的左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张期末内径(LVEDD)、心输出量(CO)、室间隔厚度(IVS)、左心室质量(LV mass).采用Pearson相关分析CHD患者血清IL-5、GPX4水平与心功能指标的相关性.结果 疾病组收缩压、低密度脂蛋白胆固醇(LDL-C)水平明显高于对照组,而血清IL-5、GPX4、高密度脂蛋白胆固醇(HDL-C)水平明显低于对照组,差异均有统计学意义(P<0.05).疾病组LVESD、LVEDD、LV mass、IVS明显大于对照组,血清BNP、CK-MB水平明显高于对照组,而LVEF、CO明显低于对照组,差异均有统计学意义(P<0.05).NYHA分级心功能Ⅳ级CHD患者LVEF、CO及血清IL-5、GPX4水平明显低于Ⅲ级、Ⅱ级患者,且Ⅲ级CHD患者LVEF、CO及血清IL-5、GPX4水平明显低于Ⅱ级患者,差异均有统计学意义(P<0.05);NYHA分级心功能Ⅳ级患者LVESD、LVEDD、LV mass、IVS明显大于Ⅲ级、Ⅱ级患者,血清BNP、CK-MB水平明显高于Ⅲ级、Ⅱ级患者,差异均有统计学意义(P<0.05);Ⅲ级CHD患者LVESD、LVEDD、LV mass、IVS大于Ⅱ级患者,血清BNP、CK-MB水平明显高于Ⅱ级患者,差异均有统计学意义(P<0.05).Pearson相关分析结果显示,CHD患者血清IL-5、GPX4水平与血清BNP、CK-MB水平和LVESD、LVEDD、LV mass、IVS均呈负相关(P<0.05),与LVEF、CO均呈正相关(P<0.05).结论 CHD患者血清IL-5、GPX4水平均明显降低,且二者均与心功能指标有密切联系.

Objective To explore the correlation between the levels of serum interleukin-5(IL-5),glutathi-one peroxidase-4(GPX4)and cardiac function in patients with coronary atherosclerotic heart disease(CHD).Methods A total of 126 patients with CHD who were admitted to Dongcheng District First People's Hospital from October 2021 to October 2022 were prospective selected as the disease group.Among them,there were 36 cases of grade Ⅱ,62 cases of grade Ⅲ and 28 cases of grade Ⅳ according to the New York Heart Associa-tion(NYHA)classification.Another 120 healthy individuals who underwent physical examinations at the Dongcheng District First People's Hospital during the same period were selected as the control group.Baseline data of CHD patients were collected.The levels of IL-5,GPX4,brain natriuretic peptide(BNP)and creatine kinase isoenzyme(CK-MB)in the serum of each group were detected by enzyme-linked immunosorbent as-say.The left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD),left ven-tricular end-diastolic diameter(LVEDD),cardiac output(CO),interventricular septum thickness(IVS),and left ventricular mass(LV mass)of patients with CHD were diagnosed by color Doppler ultrasound.Pearson correlation analysis was applied to analyze the correlation between serum IL-5,GPX4 and cardiac function in-dicators in CHD patients.Results The systolic blood pressure and low-density lipoprotein cholesterol(LDL-C)level in the disease group were significantly higher than those in the control group,while the serum levels of IL-5,GPX4 and high-density lipoprotein cholesterol(HDL-C)were significantly lower than those in the control group,and the differences were all statistically significant(P<0.05).The LVESD,LVEDD,LV mass,IVS in the disease group were obviously larger than those in the control group,and serum BNP,CK-MB levels were significantly higher than those in the control group,while LVEF and CO were significantly lower than those in the control group,and the differences were all statistically significant(P<0.05).The LVEF,CO and serum levels of IL-5 and GPX4 in NYHA class Ⅳ CHD patients were significantly lower than those in patients with NYHA class Ⅲ and Ⅱ,moreover,the LVEF,CO and serum levels of IL-5 and GPX4 in patients with NYHA class Ⅲ were significantly lower than those in patients with NYHA class Ⅱ,and the differences were statistically significant(P<0.05).The LVESD,LVEDD,LV mass,IVS of patients with NYHA class Ⅳwere obviously larger than those of patients with classes Ⅲ and Ⅱ,serum BNP and CK-MB levels were signif-icantly higher than those of patients with classes Ⅲ and Ⅱ,with statistically significant differences(P<0.05);moreover,the LVESD,LVEDD,LV mass,IVS of patients with class Ⅲ CHD were larger than those of patients with class Ⅱ,and serum BNP,CK-MB levels were significantly higher than those of patients with class Ⅱ,with statistically significant differences(P<0.05).The results of Pearson correlation analysis showed that the levels of serum IL-5 and GPX4 in patients with CHD were negatively correlated with serum BNP,CK-MB levels,LVESD,LVEDD,LV mass and IVS(P<0.05),and positively correlated with LVEF and CO(P<0.05).Conclusion The expression levels of IL-5 and GPX4 in the serum of CHD patients are promi-nently reduced,and they are closely related to cardiac function indicators.
