目的:通过分析胚胎植入前遗传学检测(preimplantation genetic testing,PGT)周期中整倍体与非整倍体胚胎的形态学和动力学参数,探讨染色体非整倍性对着床前胚胎发育的影响.方法:回顾性分析PGT周期中使用时差培养箱的胚胎发育数据,统计学检验方法为秩和检验和卡方检验.结果:共纳入487个周期的1 888个胚胎,整倍体胚胎724个(38.30%),非整倍体胚胎1 164个(61.70%);就形态学参数而言,在已经发育到囊胚阶段的胚胎中,染色体组的非整倍性异常对D1及D2胚胎无影响,对D3胚胎影响不明显,但对囊胚期胚胎有明显影响,表现为Gardner评分越差,整倍体率越低;动力学参数中,PGT-A组中,当女方年龄<35岁时整倍体胚胎和非整倍体胚胎在t3、t4、CC2、t6、t7、t8、tSB、tB、tB-t8、KIDScore和iDAScore评分上的差异有统计学意义(P<0.05),当女方年龄≥35岁时差异有统计学意义的参数为t6、t7、t8、CC3、tSB、tB、tB-tSB、KIDScore和iDAScore评分;PGT-M组中,无论女方是否高龄,有统计学意义的参数只有iDAScore评分;PGT-SR组中,当女方年龄<35岁时差异有统计学意义的参数为tPNf、t6、t7、t8、CC3、tB、KIDScore和iDAScore评分,当女方年龄≥35岁时差异有统计学意义的参数为tPB2、iDAScore评分,3组表现为整倍体胚胎比非整倍体胚胎发育得更快、细胞分裂同步性更好、评分更高;没有证据表明以下单条染色体的非整倍性异常对胚胎动力学有影响,如chr3、chr7、chr8、chr10、chr11、chr12、chr13、chr15、chr18、chr20和chr21.结论:胚胎染色体组非整倍性异常对着床前胚胎的发育有影响,但不是每个阶段都有影响.此外,某些单条染色体的非整倍性异常不影响着床前胚胎发育.

Objective:To analyze the morphological and morphokinetic data of euploid embryos and aneuploid embryos in the preimplantation genetic testing(PGT)cycle,the study aims to explore the impact of chromosomal aneuploidy on the development of preimplantation embryos.Methods:A retrospective analysis was conducted on the developmental data of embryos that under-went PGT and were cultured in time-lapse incubators.Statistical methods,including the Mann-Whitney U test and Chi-Square test,were used to analyze the data.Results:A total of 1888 embryos form 487 clinical cycles were included,including euploid em-bryos were 724(38.30%)and aneuploid embryos were 1 164(61.70%).In embryos that had developed to the blastocyst stage,chromosomal aneuploidy had no impact on D1 and D2 embryos,and had moderate effect on D3 embryos,but had significant im-pact on blastocyst stage embryos,manifested as the worse the Gardner score,the lower the euploidy rate.In terms of morphokinet-ic parameters,In the PGT-A group,when the female age was<35 years old,the parameters with statistically significant differ-ences were t3,t4,CC2,t6,t7,t8,tSB,tB,tB-t8,KIDScore and iDAScore between euploid embryos and aneuploid embryos were statistically significant(P<0.05).When the female age was≥35 years old,the parameters with statistically significant dif-ferences were t6,t7,t8,CC3,tSB,tB,tB-tSB,KIDScore and iDAScore.In the PGT-M group,regardless of whether the fe-male was of advanced age or not,only the iDAScore was statistically significant.In the PGT-SR group,when the female age was<35 years old,the parameters with statistically significant differences were tPNf,t6,t7,t8,CC3,tB,KIDScore and iDAS-core.When the female age was≥35 years old,the parameters with statistically significant differences were tPB2 and iDAScore.All three groups showed that euploid embryos developed faster,had better cell division synchrony than aneuploid embryos,also re-ceived higher scores.Single chromosome aneuploidies of chr-3,chr-7,chr-8,chr-10,chr-11,chr-12,chr-13,chr-15,chr-18,chr-20,and chr-21,had not influence on the morphokinetic of embryos in their preimplantation development.Conclusion:Chro-mosome aneuploidy in embryos has an impact on preimplantation embryo development,but not at every stage.Additionally,some single chromosome aneuploidy abnormalities do not affect preimplantation embryo development.
