胰腺假性囊肿空肠吻合术后脾动脉假性动脉瘤破裂出血一例OA
Hemorrhage of splenic artery pseudoaneurysm after jejunostomy of pancreatic pseudocyst:A case report
A 51-year-old female with a history of severe acute pancreatitis developed pancreatic pseudocysts(PPC)and an associated splenic artery pseudoaneurysm(SAPA).She underwent PPC jejunostomy Roux-en-Y anastomosis.Intraoperatively,a 5.0 mm pulsatile nodule suggestive of SAPA was identified but not treated due to severe inflammatory adhesions.On postoperative day 5,the patient experienced life-threatening hemorrhage due to SAPA rupture.Emergency laparotomy with suture ligation followed by selective splenic artery embolization successfully controlled the bleeding.The patient recovered well with no recurrence of PPC or complications during 3-year follow-up.This case underscores the importance of preoperative vascular assessment and proactive management of SAPA when planning internal drainage procedures.
