Development of new type of pressure detector with endotracheal tube for airbag and its clinical application

目的:研发自制新型气管导管气囊压力检测器并评价其在降低术后并发症的临床应用效果.方法:以430系统单片机为控制核心与高性能压力传感器MPX5010以及国产低能耗微型气泵等构建气管导管气囊压力检测器;检测新型气管导管气囊压力,通过单片机连接测压模块,连续检测气管导管气囊压力.选取2023年2月至2024年2月武汉市中医医院及华中科技大学同济医学院附属同济医院收治的244例胃肠外科手术患者,采用随机数表法将其分为对照组和观察组,每组122例.两组均接受常规手术治疗,术中对照组由具有5年以上工作经验的麻醉医生根据临床经验(手指触感法)判断并维持导管气囊压力,观察组采用新型气管导管气囊压力检测器检测气囊压力.两组均使用标准气囊压力(20~30 cmH2O)计进行气囊压力验证,对不同时间段(即插管后即刻、术中定时、拔管前)的气管导管相关并发症、气管导管气囊压力以及预后各项指标进行对比.结果:术后24 h观察组出现咳嗽38例、咽喉疼痛26例,其发生率显著低于对照组,两组比较差异有统计学意义(x2=11.388、12.739,P<0.05);术后1个月,观察组均未出现声音嘶哑,对照组声音嘶哑4例,观察组发生率显著低于对照组,两组比较差异有统计学意义(x2=4.067,P<0.05);观察组插管后即刻、术中定时及拔管前的气管导管气囊压力正常率分别是90.16%、90.98%和91.80%,两组比较差异均有统计学意义(x2=11.091、10.583、11.017,P<0.05);观察组插管后即刻、术中定时及拔管前气囊压力分别是(38.36±4.25)、(28.36±5.54)和(24.55±2.55)cmH2O,显著低于对照组,差异均有统计学意义(t=10.366、12.366、11.544,P<0.05).观察组患者治疗满意度为91.80%,对照组患者治疗满意度为75.41%,观察组患者治疗满意度显著高于对照组,差异有统计学意义(x2=11.167,P<0.05);40名医务人员对观察组气道压力测量方法满意度为90.00%,对照组满意率为67.50%,差异有统计学意义(x2=6.050,P<0.05).结论:新型气管导管气囊压力检测器的研究符合临床实际要求,其自动监测和调控功能可准确检测气囊压力,患者不同时间段的气管导管相关并发症显著减少,气囊压力值趋于正常,减少患者住院时间,提升患者和医护人员满意度.

Objective:To evaluate the development of a self-made new type of pressure detector with endotracheal tube for airbag and its clinical practice and application effect in reducing postoperative complications.Methods:The airbag's pressure of new type of pressure detector with endotracheal tube for airbag was detected,which was connected with module of detecting pressure through single chip machine,and the pressure was continuously detected.A total of 244 patients,who underwent gastrointestinal surgery at Wuhan Hospital of Traditional Chinese Medicine and Tongji Hospital,Tongji Medical College of Huazhong University of Science and Technology from February 2023 to February 2024,were selected,and they were divided into a control group and an observation group by random number table method,with 122 cases in each group.Both groups received conventional surgical treatment,the airbag's pressures of the tubes of patients of control group were judged and maintained by anesthesiologists with more than 5 years of working experience according to their clinical experience(finger touch method)during surgery period,and the observation group adopted the new type of pressure detector with endotracheal tube for airbag to detect airbag's pressure.Both groups used standard pressure gauge(20-30 cmH2O)for airbag to verify the pressure of airbag.The endotracheal tube-related complications,airbag's pressure of the endotracheal tube,and various prognostic indicators among different time stages(instant after intubation,at intraoperatively fixed time,and the time before extubation)were compared.Results:At the 24th hour after the operation,38 cases occurred cough and 26 cases occurred throat pain in the observation group,which incidence rates were significantly lower than those in the control group,and the differences of them between the two groups were statistically significant(x2=11.388,12.739,P<0.05).In one month after the operation,there was not hoarseness in the observation group,while 4 cases occurred hoarseness in the control group,and the incidence rate of the observation group was significantly lower than that of the control group,and the difference between the two groups was statistically significant(x2=4.067,P<0.05).The normal rates of airbag's pressure of endotracheal tube at instant after intubation,at intraoperatively fixed time,and the time before extubation were respectively 90.16%,90.98%,and 91.80%in the observation group,and there were statistically significant differences between the two groups(x2=11.091,10.583,11.017,P<0.05).The airbag's pressures at instant after intubation,at intraoperatively fixed time,and the time before extubation were respectively(38.36±4.25),(28.36±5.54)and(24.55±2.55)cmH2O in the observation group,which were significantly lower than those in the control group,and the differences were statistically significant(t=10.366,12.366,11.544,P<0.05).The curative satisfaction degree of patients in the observation group was 91.80%,which was significantly higher than 75.41%of patients in the control group,and the difference was statistically significant(x2=11.167,P<0.05).The satisfaction degree of 40 medical staffs for the usage of equipment in the observation group was 90.00%,and for that in the control group was 67.50%,and the difference was statistically significant(x2=6.050,P<0.05).Conclusion:The research about the new type of pressure detector with endotracheal tube for airbag's pressure can meet the clinically actual requirements.Its functions in automatic monitoring and regulation can accurately detect airbag's pressure.The endotracheal tube-related complications of patients at different time stages were significantly reduced,and the airbag's pressure value tends to be normal,and the hospital stay of patients is shortened,and the satisfaction of patients and medical staffs is improved.
