基于脑卒中Brunnstrom分期的下肢机器人精准剂量训练在足下垂康复中的应用观察OA
Application of precision dosing training of lower limb robot based on Brunnstrom staging in rehabilitation of foot drop after stroke
目的:探讨基于Brunnstrom分期的下肢康复机器人精准剂量训练对脑卒中后足下垂患者的康复疗效.方法:选取2021年3月至2023年6月期间六安市人民医院康复医学中心收治的100例Brunnstrom Ⅲ~Ⅳ期脑卒中患者,按照随机数表法将其分为观察组(49例)和对照组(51例).观察组采用机器人步态训练系统(Lokomat Pro)进行分期动态调整训练,对照组实施标准化康复方案训练.评估两组干预前后踝背屈活动度(AROM)、步行功能及安全性指标.结果:干预后观察组AROM提升(8.62±2.15)°,显著高于对照组的(4.73±1.84)°,差异有统计学意义(t=6.327,P<0.05);10 m步速增幅达(0.28±0.09)m/s,较对照组高0.13 m/s,差异有统计学意义(t=8.135,P<0.05);Fugl-Meyer评估量表评分提升(10.07±3.84)分,显著优于对照组的(5.22±2.67)分,差异有统计学意义(t=6.724,P<0.05).Holden功能步行分级显示观察组独立行走率(63.27%)显著高于对照组(37.25%),差异有统计学意义(Z=3.127,P<0.05).安全性方面观察组不良事件发生率为26.53%,显著低于对照组的58.82%,差异有统计学意义(x2=10.367,P<0.05).结论:Brunnstrom分期导向的机器人精准剂量训练可显著改善脑卒中足下垂患者的运动功能,其疗效优势体现在踝关节活动度提升、步行速度改善及独立行走率提高,且具有更好的安全性.
Objective:To investigate the rehabilitation efficacy of precision dosing training of lower limb rehabilitation robot based on Brunnstrom staging on patients with food drop after stroke.Methods:A total of 100 patients with stroke at Brunnstrom stages Ⅲ-Ⅳ,who admitted to the Rehabilitation Medicine Center of Lu'an People's Hospital between March 2021 and June 2023,were enrolled.They were randomly divided into observation group(49 cases)and control group(51 cases)according to random number table.The observation group adopted robotic gait training system(Lokomat Pro)to conduct staged dynamic adjustment training,while the control group adopted standardized rehabilitation plan to conduct training.Ankle dorsiflexion active range of motion(AROM),walking function,and safety indicators before and after the intervention were assessed.Results:After the intervention,the enhance value of AROM of the observation group was(8.62±2.15)°,which was significantly greater than(4.73±1.84)° of the control group,and the difference was significant(t=6.327,P<0.05).The increased value of 10-meter walking speed was(0.28±0.09)m/s in the observation group,which was 0.13 m/s higher than that in the control group,with statistically significant difference(t=8.135,P<0.05).The score of Fugl-Meyer scale improved by(10.07±3.84)points in the observation group,which was significantly superior to(5.22±2.67)points of the control group(t=6.724,P<0.05).According to the Holden Functional Ambulation Classification,the rate of independent walking(63.27%)in the observation group was significantly higher than 37.25%in the control group,with statistically significant difference(Z=3.127,P<0.05).In terms of safety,the incidence of adverse events in the observation group was 26.53%,which was significantly lower than 58.82%in the control group(x2=10.367,P<0.05).Conclusion:Brunnstrom staging-guided precision dosage training of robot can significantly improve motor function of patients with foot drop caused by stroke.The therapeutic advantages appear enhancement of ankle mobility,improvement of walking speed,increase of independent walking capacity,and superior safety.
