基于Wnt/β-catenin通路及炎症因子的种植体周围感染风险预测模型构建及效能验证OA
Development and validation of a predictive model for peri-implant infection risk using Wnt/β-catenin pathway and inflammatory factors
目的:探讨种植体周围感染影响因素,基于Wnt/β-catenin通路因子建立种植体周围感染风险预测模型并进行效能验证.方法:前瞻性选取2022 年1 月~2024 年6 月收治的种植义齿患者239 例,根据种植修复术后12个月内是否并发种植体周围感染,分为感染组(n=51)与未感染组(n=188).比较两组患者临床相关资料及Wnt/β-catenin通路相关因子水平差异.采用二元Logistic回归分析种植体周围感染影响因素,并构建风险预测模型.建立受试者工作特征(receiver operating characteristic,ROC)曲线分析Wnt mRNA、β-catenin mRNA、白介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)对种植体周围感染的预测效能.结果:两组吸烟史、糖尿病比较,差异有统计学意义(P<0.05);感染组Wnt mRNA、β-catenin mRNA均低于未感染组,IL-6、TNF-α均高于未感染组(P<0.05).二元Logistic回归分析,Hosmer-Lemeshow检验结果提示联合预测模型拟合度良好.糖尿病、低Wnt mRNA、低β-catenin mRNA、高IL-6、高TNF-α为种植体周围感染独立危险因素(P<0.05).糖尿病、Wnt mRNA、β-catenin mRNA、IL-6、TNF-α联合预测曲线下面积(area under the curve,AUC)均高于各单项因素(P<0.05).结论:Wnt/β-catenin通路相关因子及糖尿病均为种植体周围感染的独立危险因素,联合构建的风险预测模型对种植体周围感染有较高预测效能.
Objective:To investigate the influencing factors of peri-implant infection,and establish a risk prediction model for peri-implant infection based on Wnt/β-catenin pathway factors and verify its efficacy.Methods:Totally 239 pa-tients with dental implants from January 2022 to June 2024 were prospectively selected and divided into the infection group(n=51)and the non-infection group(n=188)based on whether peri-implant infection occurred within 12 months after im-plant restoration.Compare the clinical relevant data and the differences in the levels of Wnt/β-catenin pathway-related factors between the two groups of patients.Binary Logistic regression was used to analyze the influencing factors of peri-implant in-fection in patients with implant denture,and a risk prediction model was constructed.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of Wnt mRNA,β-catenin mRNA,interleukin-6(IL-6)and tumor necro-sis factor-alpha(TNF-α)for peri-implant infection.Results:There was a statistically significant difference in smoking history and diabetes between the two groups(P<0.05).The Wnt mRNA and β-catenin mRNA in the infection group were lower than those in the non-infection group,while the IL-6 and TNF-α were higher(P<0.05).Binary Logistic regression analysis showed that the Hosmer-Lemeshow test result indicating a good fit of the joint prediction model.Diabetes mellitus,low Wnt mRNA,low β-catenin mRNA,high IL-6,and high TNF-α were independent risk factors for peri-implant infection in patients with im-plant denture(P<0.05).The AUC of the diabetes,Wnt mRNA,β-catenin mRNA,IL-6,TNF-α combined prediction was higher than that of each individual factor(P<0.05).Conclusion:Wnt/β-catenin pathway related factors and diabetes melli-tus are independent risk factors for peri-implant infection in patients with implant-supported dentures.The risk prediction model jointly constructed has a high prediction efficiency for peri-implant infection.
