目的:应用MRI图像纹理分析技术评估单侧第三磨牙伸长患者翼外肌的改变状况及明确单侧第三磨牙伸长与翼外肌变化的相关关系.方法:对2020 年10 月开始于新疆医科大学第一附属医院附属口腔医院颞下颌关节专病门诊就诊的 28 例单侧第三磨牙伸长患者行颞下颌关节MRI检查.按单侧第三磨牙伸长的侧别分为伸长侧组和非伸长侧组.采用灰度共生矩阵法(gray-level co-occurrence matrix,GLCM)对闭口位时翼外肌分别进行角二阶矩、对比度、相关性、逆差距、熵等 5 种纹理特征参数测量并分析.结果:伸长侧组与非伸长侧组翼外肌的角二阶矩、对比度、逆差距、熵均有统计学差异(P<0.05);利用受试者工作特征(receiver operating characteristic,ROC)曲线分析,熵对评估单侧第三磨牙伸长患者翼外肌改变的能力最佳,曲线下面积(area under the curve,AUC)为 0.702,约登指数为 0.429,最佳截断值为 7.565,评估翼外肌改变的敏感性和特异性分别为 64.3%和 78.6%.结论:纹理特征参数熵可以初步用以评估单侧第三磨牙伸长患者翼外肌的改变状况.

Objective:To evaluate the changes of lateral pterygoid muscle in patients with unilateral over-erupted third molar by MRI image texture analysis and to clarify the correlation between unilateral over-erupted third molar and lateral pterygoid muscle.Methods:A total of 28 patients with unilateral over-erupted third molar,were enrolled in the outpatient clinic of the First Affiliated Hospital&Stomatological Hospital of Xinjiang Medical University from October 2020.According to the over-erupted side of the unilateral third molar,the patients were divided into two groups:the over-erupted side group and the non-over-erupted side group.The gray level co-occurrence matrix(GLCM)was used to measure the texture parameters of lateral pterygoid muscle in closed position,including angular second moment,contrast,correlation,inverse difference mo-ment and entropy,to compare the changes of lateral pterygoid muscle between over-erupted side group and non-over-erupted side group in patients with unilateral over-erupted third molar.Results:There was statistically significant in angular second moment,contrast,inverse difference moment and entropy of lateral pterygoid muscle between the over-erupted side group and the non-over-erupted side group(P<0.05).The area under the ROC curve(area under the curve,AUC)of entropy for evalua-ting the changes of lateral pterygoid muscle in patients with unilateral over-erupted third molar was 0.702,the Yoden index was 0.429,and the best truncation value was 7.565.The sensitivity and specificity of evaluating lateral pterygoid muscle changes were 64.3%and 78.6%.Conclusion:The texture parameter of entropy can be used to evaluate the changes of later-al pterygoid muscle in patients with unilateral over-erupted third molar.
