目的:探究基于加速康复外科理念(ERAS)的综合干预对扁桃体炎切除术患者术后疼痛、心理健康及生活质量的影响.方法:研究采用便利抽样法,于2023年3月至2024年3月期间纳入某院耳鼻喉科收治的扁桃体切除患者104例作为观察对象,通过随机数字表法均分为研究组与对照组各52例.对照组予以常规干预方案,在此基础上研究组予以基于ERAS的综合干预,干预时间为1个月.采用疼痛视觉模拟评分(VAS)、自评焦虑量表(SAS)、自评抑郁量表(SDS)、心理痛苦温度计(DT)、欧洲癌症研究与治疗组织生活质量问卷(EORTC QLQ-C30)对两组患者干预效果进行评价.结果:干预前两组VAS、SAS、SDS、DT、生活质量各个维度评分差异无统计学意义(P>0.05).干预后研究组患者VAS评分显著低于对照组(t=-7.305,P<0.05);干预后研究组患者SAS和SDS评分显著低于对照组(t=-8.726,-11.088;P<0.05);干预后研究组患者DT评分显著低于对照组(t=-2.918,P<0.05);干预后研究组患者功能领域、整体生活质量领域评分显著高于对照组(t=5.727,6.599;P<0.05);症状领域、单项项目评分显著低于对照组(t=-3.059,-8.016;P<0.05),差异均有统计学意义.结论:ERAS的综合干预可有效缓解扁桃体炎切除术患者术后疼痛,同时促进其心理健康状态的恢复和生活质量的提升.

Objective:To investigate the clinical efficacy of an Enhanced Recovery After Surgery(ERAS)—based comprehensive intervention in improving postoperative pain control,psychological adjustment,and holistic recovery outcomes among tonsillectomy patients diagnosed with tonsillitis.Methods:A convenience sampling approach was used to recruit 104 patients who underwent tonsillectomy in the otorhinolaryngology department of our hospital be-tween March 2023 and March 2024.Participants were randomly assigned to a study group(n=52)and a control group(n=52)using a random number table.The control group received conventional perioperative care,while the study group received an ERAS-based comprehensive intervention in addition to routine care.The intervention period lasted 1 month.Outcomes were assessed using the Visual Analog Scale(VAS)for pain,the Self-Rating Anxiety Scale(SAS),the Self-Rating Depression Scale(SDS),the Distress Thermometer(DT),and the European Organiza-tion for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30).Subdomains of the EORTC QLQ-C30 were analyzed under functional status,symptom burden,and global health status categories.Re-sults:At baseline,the two groups demonstrated comparable scores in VAS,SAS,SDS,DT,and all quality—of-life dimensions(P>0.05).Post-intervention,the VAS score in the study group was significantly lower than in the con-trol group(t=-7.305,P<0.05).The SAS and SDS scores in the study group were significantly lower than in the control group(t=-8.726,-11.088;P<0.05).The DT score in the study group was significantly lower than in the control group(t=-2.918,P<0.05).The Functional Scale and Global Quality of Life Scale scores in the study group were significantly higher than in the control group(t=5.727,6.599;P<0.05).The Symptom Scale and Single Item scores in the study group were significantly lower than in the control group(t=-3.059,-8.016;P<0.05).Conclusion:The ERAS-based comprehensive intervention effectively alleviates postoperative pain in patients undergoing tonsil-lectomy,improved psychological well-being,and enhances overall quality of life.These findings highlight its promis-ing clinical utility in perioperative management.
