近五年针刺治疗慢性失眠障碍的作用机制研究进展OA
Research advances in the mechanism of acupuncture for treating chronic insomnia disorder over the past five years
慢性失眠障碍(chronic insomnia,CI)以入睡困难、睡眠质量下降且持续时间超过 3个月为特征,严重影响身心健康.本文分析近年相关文献,发现针刺治疗CI具有以下作用机制:(1)调节神经递质.单胺类神经递质中,针刺能降低促进觉醒的多巴胺D1 受体活性及去甲肾上腺素含量,同时提升对睡眠至关重要的5-羟色胺及其受体水平,从而改善睡眠—觉醒周期.氨基酸类递质中,针刺可升高 γ-氨基丁酸含量、降低谷氨酸含量,调节神经系统兴奋与抑制平衡.(2)调节神经内分泌激素.CI与下丘脑—垂体—肾上腺(hypothalamic-pituitary-adrenal,HPA)轴过度激活、褪黑素分泌节律紊乱有关.HPA轴过度激活会使促肾上腺皮质激素释放激素、促肾上腺皮质激素、皮质醇浓度升高,干扰睡眠觉醒周期.针刺可降低这些激素含量,抑制HPA轴过度应激反应.褪黑素分泌水平降低会导致睡眠质量下降,针刺可提高褪黑素含量,改善睡眠质量.(3)调节生物钟蛋白转录.CI患者常表现为中枢钟与外周钟节律失同步,针刺可调节生物钟核心蛋白,如上调昼夜节律运动输出周期故障和芳基烃受体核转运体样蛋白的mRNA表达水平、提高PER1、PER2 mRNA表达等,重塑中枢—外周钟的协同振荡,恢复睡眠—觉醒周期.尽管针刺治疗CI疗效确切,但现有研究仍存在不足,如部分机制尚不明确、多集中单一机制研究、实验样本量少等.未来研究可深入探讨针刺治疗CI的作用机制,挖掘多个靶点间相互作用,增加临床试验样本量,为针刺治疗CI提供新思路.
Chronic insomnia disorder(CI)is characterised by difficulty in falling asleep and reduced sleep quality lasting more than 3 months,which seriously affects physical and mental health.By analysing the literature in recent years,it was found that acupuncture treatment for CI has the following mechanisms of action:(1)Modulation of neurotransmitters.Among the monoamine neurotransmitters,acupuncture can reduce the activity of dopamine D1 receptor and the content of norepinephrine(NE),which promotes arousal,and at the same time increase the level of 5-hydroxytryptophan(5-HT)and its receptor,which is essential for sleep,thus improving the sleep-wake cycle.Among the amino acid transmitters,acupuncture can elevate GABA content and lower Glu content,regulating the balance of excitation and inhibition in the nervous system.(2)In the regulation of neuroendocrine hormones,CI is related to the over-activation of the HPA axis and the disruption of the MT secretion rhythm;the over-activation of the HPA axis will increase the concentration of CRH,ACTH and CORT,which will interfere with the sleep-wake cycle,and acupuncture can reduce the content of these hormones and inhibit the over-stress response of the HPA axis;the decrease in the secretion level of MT will lead to the decrease of sleep quality,and acupuncture can increase the content of MT and improve the quality of sleep.(3)Transcription of biological clock proteins.Chronic insomnia patients often exhibit desynchronisation of the central and peripheral clock rhythms,and acupuncture can reshape the synergistic oscillations of the central-peripheral clock and restore the sleep-wake cycle by regulating the core proteins of the biological clock,such as up-regulation of the expression levels of Clock and Bmal1 mRNA,and increasing the expression of Per1,Per2 mRNA,and so on.Despite the potential of acupuncture in treating CI,there are shortcomings in existing studies,such as some of the mechanisms are still unclear,mostly focusing on single-mechanism studies,and small experimental sample sizes.Future studies can explore the mechanism of action of acupuncture for CI in depth,tap into the interactions between multiple targets,increase the sample size of clinical trials,and provide new ideas for the treatment of CI with acupuncture.
