多种肿瘤相关自身抗体对肺癌的诊断价值OA

Clinical Significance of Concurrent Identification of Various Tumor-Associated Autoantibodies in Lung Cancer Diagnosis

目的:探讨血清中多种肿瘤相关自身抗体的单独检测及其联合检测在肺癌临床诊断中的应用价值.方法:选取2022年1 月—2023 年 6 月南宁市第一人民医院收治的 97 例诊断为肺癌的患者为肺癌组,同期 82 例诊断为非肺癌恶性肿瘤患者为非肺癌组,80 例肺部良性病变患者为良性病变组,以及 82 名体检健康者且无明显肺部异常者为对照组.采用酶联免疫吸附试验(ELISA)检测 4 组研究对象血清中多种肿瘤相关自身抗体:P53 蛋白(P53)、神经元特异性蛋白(PGP9.5)、SRY-box转录因子-2(SOX2)、肿瘤相关睾丸抗原 7(GAGE7)、RNA解旋酶抗体 4-5(GBU4-5)、素瘤相关抗原A1(MAGE A1)和癌症相关抗原(CAGE)表达水平.比较 4 组研究对象的P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGE A1、CAGE表达水平及联合检测的阳性率.分析P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGE A1、CAGE水平及联合检测在肺癌患者中的诊断价值.结果:肺癌组P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGE A1、CAGE水平高于对照组,差异有统计学意义(P<0.05);肺癌组与非肺癌组比较,除P53 外,PGP9.5、SOX2、GAGE7、GBU4-5、MAGE A1、CAGE表达水平比较,差异无统计学意义(P>0.05);肺癌组与肺部良性疾病组相比,除P53、PGP9.5 外,其余上述抗体表达水平比较,差异无统计学意义(P>0.05).联合检测的阳性率分别为肺癌组 49.8%、非肺癌组 26.8%、肺部良性病变组 16.25%,差异有统计学意义(P<0.05).受试者工作特征(ROC)曲线分析显示联合检测肺癌的敏感度为 69.1%,特异度为 72.1%,ROC曲线下面积(AUC)为 0.749,差异有统计学意义(P<0.05).结论:多种肿瘤相关自身抗体联合检测对肺癌具有一定的诊断价值.

Objective:The research on the individual detection of various tumor-associated autoantibodies in serum and their combined detection in the clinical diagnostic value of lung cancer.Method:Totally 97 patients with a diagnosis of lung cancer,who were admitted to the First People's Hospital of Nanning from January 2022 to June 2023,were enrolled as the lung cancer group,82 patients diagnosed with non-lung cancer malignant tumors during the same period were selected as the non-lung cancer group,80 patients with benign lung lesions were selected as the benign lesion group,and 82 healthy individuals with no obvious lung abnormalities during physical examination were selected as the control group.Using the enzyme-linked immunosorbent assay(ELISA)to detect various tumor-associated autoantibodies in the serum of four groups of study subjects:P53 protein(P53),neuron-specific protein(PGP9.5),sex-determining region Y gene 2(SOX2),tumor-associated testis antigen 7(GAGE7),RNA helicase autoantibody 4-5(GBU4-5),melanoma-associated antigen A1(MAGE A1),and cancer-associated antigen(CAGE)expression levels.Comparing the expression levels of P53,PGP9.5,SOX2,GAGE7,GBU4-5,MAGE A1,and CAGE and the positive rate of combined detection among the four groups of study subjects.Analyzing the diagnostic value of P53,PGP9.5,SOX2,GAGE7,GBU4-5,MAGE A1,CAGE levels and combined detection in lung cancer patients.Result:The levels of P53,PGP9.5,SOX2,GAGE7,GBU4-5,MAGE A1,and CAGE in the lung cancer group were higher than the control group,showing significant differences(P<0.05).Relative to the non-lung cancer malignant tumor group,the lung cancer group revealed no statistically significant differences in the expression levels of PGP9.5,SOX2,GAGE7,GBU4-5,MAGE A1,and CAGE,apart from P53(P>0.05).Relative to the benign lung disease group,there were no statistically significant differences in the expression levels of the other autoantibodies previously mentioned in the lung cancer group,except for P53 and PGP9.5(P>0.05).The combined detection positive rates were 49.8%in the lung cancer group,26.8%in the Non-Lung Cancer Group,and 16.25%in the Benign Pulmonary Lesions Group,with statistically significant differences among the groups(P<0.05).Receiver Operating Characteristic(ROC)curve analysis revealed that the combined detection for lung cancer had a sensitivity of 69.1%,a specificity of 72.1%,and an area under the ROC curve(AUC)of 0.749,all of which were statistically significant(P<0.05).Conclusion:The combined detection of multiple tumor-related autoantibodies has certain diagnostic value for lung cancer.
