目的:探讨新生儿坏死性小肠结肠炎(NEC)肠造瘘术后肠道菌群结构及功能改变对婴幼儿体格生长发育的影响.方法:选取 2022 年 1-12 月深圳市儿童医院收治的 22 例NEC肠造瘘术后患儿为观察组,并选取同期的 22 名健康婴幼儿为对照组.收集多个时间点粪便样本,进行 16S核糖体RNA(16S rRNA)测序分析肠道菌群,同步记录生长发育指标,分析菌群特征与生长发育的关系.结果:观察组肠道菌群多样性指数明显低于对照组,差异有统计学意义(P<0.05);关瘘术后 6 个月,观察组菌群结构逐渐恢复,与对照组比较,差异有统计学意义(P<0.05);观察组肠道菌群功能分析显示碳水化合物代谢相关基因表达减少,差异有统计学意义(P<0.01);菌群结构变化与身体质量指数(BMI)呈正相关(r=0.726,P<0.01).结论:关瘘术后恢复缓慢,菌群结构及功能改变与体格生长发育迟缓密切相关,可通过调节肠道菌群改善患儿生长发育.

Objective:To explore the influence of changes in the structure and function of intestinal flora after enterostomy for neonatal necrotizing enterocolitis(NEC)on the physical growth and development of infants and young children.Method:22 children with neonatal necrotizing enterocolitis(NEC)after enterostomy admitted to Shenzhen Children's Hospital were selected as the observation group,and 22 healthy infants and young children during the same period were selected as the control group.Fecal samples were collected at multiple time points,and 16S ribosomal RNA(16S rRNA)sequencing was performed to analyze the intestinal flora.Growth and development indicators were recorded simultaneously to analyze the relationship between the characteristics of the flora and growth and development.Result:The diversity index of intestinal flora in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Six months after fistula closure,the microbiota structure in the observation group gradually recovered.Compared with the control group,the difference was statistically significant(P<0.05).The functional analysis of the intestinal flora in the observation group showed that the expression of genes related to carbohydrate metabolism decreased,and the difference was statistically significant(P<0.01).The structural changes of the microbiota were positively correlated with the body mass index(BMI)(r=0.726,P<0.01).Conclusion:The recovery after fistula closure is slow.The changes in the structure and function of the microbiota are closely related to delayed physical growth and development.The growth and development of children can be improved by regulating the intestinal microbiota.
