电刺激皮层听觉诱发电位P1-N1在无残余听力患者中的初步观察OA
Preliminary Observation of Electrically Evoked Cortical Auditory Potential P1-N1 in Patients with No Residual Hearing
目的 通过观察无残余听力患者的人工耳蜗植入(CI)术后电刺激皮层听觉诱发电位(cortical auditory evoked potential,CAEP),为该类患者提供行CI证据.方法 选取术前主客观听力检测均显示无残余听力的5名(6耳)无残余听力语前聋儿童,于开机后进行电刺激P1-N1 CAEP测试,对植入6月以上患者记录听觉行为分级-Ⅱ(CAP-Ⅱ)、言语可懂度分级标准(SIR)量表得分.结果 5名患者(6耳)术后均能够在不同刺激强度下记录到典型的P1-N1波形反应,部分患儿还可引出P2波.P1波潜伏期范围为37.3~125.8 ms,N1波潜伏期范围为150.8~205.4 ms.植入6月以上的3名患者CAP-Ⅱ得分均≥6分,SIR得分均≥3分,表明其具备较好的听觉言语发育和沟通能力.结论 初步验证无残余听力患儿听觉通路的完整性.多数无残余听力患儿可以植入人工耳蜗.
Objective To provide evidence for cochlear implantation(CI)in patients with no residual hearing by observing electrically evoked cortical auditory evoked potentials(CAEP)after CI surgery.Methods Five children(six ears)with prelingual deafness and no residual hearing,as confirmed by preoperative objective and subjective hearing tests,were selected.Electrically evoked P1-N1 CAEP testing was conducted after device activation.For patients implanted for more than six months,scores from the Categories of Auditory Performance-Ⅱ(CAP-Ⅱ)and the Speech Intelligibility Rating(SIR)scale were recorded.Results Typical P1-N1 waveform responses were recorded in all five patients(six ears)at various stimulation levels,with P2 waves also observed in some children.The P1 latency ranged from 37.3 to 125.8 ms,and the N1 latency from 150.8 to 205.4 ms.The three patients implanted for more than six months achieved CAP-Ⅱ scores≥6 and SIR scores≥3,indicating favorable auditory and speech development as well as communication abilities.Conclusion This study preliminarily verifies the integrity of the auditory pathway in these patients.The majority of children with no residual hearing can undergo cochlear implantation.
