早期非小细胞肺癌经PD-1抑制剂与立体定向体部放疗联合治疗的疗效观察研究OA
Observational study on the therapeutic effect of PD-1 inhibitor combined with SABR in early-stage non-small cell lung cancer
目的:探讨早期非小细胞肺癌(NSCLC)经PD-1抑制剂与立体定向体部放疗(SABR)联合治疗的疗效观察.方法:选取154例NSCLC患者作为研究对象.按照随机数字表法,将其分为观察组和对照组,各77例.对照组给予SABR治疗,观察组给予PD-1抑制剂联合SABR治疗.比较两组患者临床疗效、肿瘤标志物水平、免疫功能、生活质量及不良反应.结果:观察组ORR(45.45%)及DCR(84.42%)显著高于对照组(19.48%、54.55%)(均P<0.05).治疗后,观察组CEA、CYFRA21-1及VEGF水平显著低于对照组(均P<0.05).观察组CD3+、CD4+、CD4+/CD8+及NK水平显著高于对照组(均P<0.05);CD8+显著低于对照组(P<0.05).观察组生活质量改善率(80.52%)显著高于对照组(46.75%,P<0.05).观察组总不良反应率(10.39%)显著低于对照组(22.08%,P<0.05).结论:PD-1抑制剂与SABR联合应用能够显著提高早期NSCLC患者治疗效果.
Objective:To examine the effectiveness of combining PD-1 inhibitor therapy with stereotactic abla-tive body radiotherapy(SABR)in treating patients diagnosed with early-stage non-small cell lung cancer(NSCLC).Methods:Between June 2021 and June 2024,a total of 154 NSCLC patients admitted to our hospital were enrolled in the study and randomized into two groups using a random number table:the observation group and the control group,with 77 patients in each.The control group underwent SABR treatment alone,whereas the observation group received a combination therapy of PD-1 inhibitor and SABR.Comparisons were made between the two groups regard-ing clinical efficacy,levels of tumor markers,immune status,quality of life,and occurrence of adverse reactions.Results:The observation group exhibited notably higher overall response rate(ORR)at 45.45%and disease control rate(DCR)at 84.42%,compared to 19.48%and 54.55%respectively in the control group(all P<0.05).Post-treatment,the observation group showed significantly reduced levels of CEA,CYFRA21-1,and VEGF compared to the control group(all P<0.05).Additionally,the observation group had significantly elevated levels of CD3+,CD4+,CD4+/CD8+,and NK cells,along with a lower CD8+cell count(all P<0.05).The observation group also reported a significantly higher quality of life improvement rate at 80.52%,compared to 46.75%in the control group(P<0.05).Furthermore,the total incidence of adverse reactions was significantly lower in the observation group at 10.39%,compared to 22.08%in the control group(P<0.05).Conclusion:The combination of PD-1 inhibitors and SABR can significantly improve the treatment efficacy of early NSCLC patients.
