基于功能性近红外光谱技术探讨疏肝调神针法对广泛性焦虑障碍患者前额叶功能的影响OA
Exploring the effect of the Shu Gan Tiao Shen acupuncture method on prefrontal cortex function in patients with generalized anxiety disorder based on functional near-infrared spectroscopy
目的 基于功能性近红外光谱技术探讨疏肝调神针刺法对广泛性焦虑障碍(generalized anxiety disorder,GAD)患者前额叶功能的调节作用.方法 将 40例GAD患者随机分为针刺组和药物组,每组20 例.同时招募健康人 20 例为健康组.针刺组予以疏肝调神针法治疗,药物组予以口服艾司西酞普兰片治疗.观察针刺组和药物组治疗前后汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分的变化,比较健康组和GAD患者、针刺组治疗前后、药物组治疗前后及针刺组和药物组治疗后前额叶氧合血红蛋白(oxyhemoglobin,HbO2)浓度变化.结果 治疗后,针刺组和药物组HAMA评分均降低(P<0.05),且针刺组低于药物组(P<0.05).GAD患者左背外侧前额叶、前额叶眶额区、双侧额极、右腹外侧前额叶脑区HbO2 浓度值低于健康组(P<0.05).药物组治疗后双侧额极、前额叶眶额区的 HbO2 浓度值升高(P<0.05);左背外侧前额叶的 HbO2 浓度值降低(P<0.05).针刺组治疗后双侧额极、前额叶眶额区、背外侧前额叶的HBO2浓度值较治疗前升高(P<0.05).针刺组治疗后背外侧前额叶、左腹外侧前额叶的HbO2 浓度值高于药物组(P<0.05).结论 前额叶在情绪调节中具有重要作用,疏肝调神针刺通过调节前额叶功能发挥治疗GAD的作用.
Objective To investigate the regulatory effect of the Shu Gan Tiao Shen acupuncture method on prefrontal cortex function in patients with generalized anxiety disorder(GAD)using functional near-infrared spectroscopy(fNIRS).Method Forty patients with GAD were randomly divided into an acupuncture group and a medication group(20 cases each).Twenty healthy subjects were recruited as the healthy group.The acupuncture group received treatment with the Shu Gan Tiao Shen acupuncture method,while the medication group received oral escitalopram tablets.The Hamilton anxiety scale(HAMA)scores before and after treatment were compared between the two treatment groups.Changes in oxyhemoglobin(HbO2)concentration in the prefrontal cortex were analyzed between healthy control and GAD patients before treatment,acupuncture group before and after treatment,medication group before and after treatment,and acupuncture group and medication group after treatment.Result After treatment,HAMA scores significantly decreased in both the acupuncture and medication groups(P<0.05),with the acupuncture group showing lower scores than the medication group(P<0.05).Compared with healthy controls,GAD patients had lower HbO2 concentrations in the left dorsolateral prefrontal cortex,orbitofrontal cortex,bilateral frontopolar regions,and right ventrolateral prefrontal cortex(P<0.05).After treatment,the medication group showed increased HbO2 concentrations in the bilateral frontopolar and orbitofrontal regions(P<0.05),while the left dorsolateral prefrontal HbO2 concentration decreased(P<0.05).After treatment,the acupuncture group demonstrated increased HbO2 concentrations in the bilateral frontopolar,orbitofrontal,and dorsolateral prefrontal regions(P<0.05).Moreover,HbO2 concentrations in the dorsolateral and left ventrolateral prefrontal cortices were higher in the acupuncture group than in the medication group after treatment(P<0.05).Conclusion The prefrontal cortex plays a crucial role in emotional regulation.The Shu Gan Tiao Shen acupuncture method may exert its therapeutic effects on GAD by modulating prefrontal cortex function.
