目的 探讨早期胃癌组织中磷酸二酯酶1B(PDE1B)、脑和肌肉芳烃受体核转运样蛋白1(BMAL1)蛋白表达情况与经内镜黏膜下剥离术(ESD)后复发的关系.方法 选取2018年1月至2021年12月在该院接受ESD治疗的155例早期胃癌患者作为研究对象.术后随访3年,根据ESD后是否复发将所有胃癌患者分为复发组和无复发组.采用免疫组织化学法检测胃癌组织及癌旁组织中PDE1B、BMAL1蛋白表达情况.通过K-M生存曲线比较PDE1B、BMAL1蛋白表达阳性、阴性的胃癌患者ESD后无复发生存率,并进行Log-rank χ2 检验.采用多因素Cox回归分析胃癌患者ESD后复发的影响因素.结果 胃癌组织中PDE1B蛋白表达阳性率高于癌旁组织,BMAL1蛋白表达阳性率低于癌旁组织,差异均有统计学意义(P<0.05).随访3年,155例胃癌患者ESD后复发45例(29.03%).复发组癌组织中PDE1B蛋白表达阳性率高于无复发组,BMAL1蛋白表达阳性率低于无复发组,差异均有统计学意义(P<0.05).K-M生存曲线分析结果显示,癌组织中PDE1B蛋白表达阳性患者ESD后无复发生存曲线整体低于PDE1B蛋白表达阴性患者(Log-rank χ2=23.290,P<0.001),癌组织中BMAL1蛋白表达阳性患者ESD后无复发生存曲线整体高于BMAL1蛋白表达阴性(Log-rank χ2=7.676,P=0.006).多因素Cox回归分析结果显示,低分化、癌组织中PDE1B蛋白表达阳性均为胃癌患者ESD后复发的独立危险因素(P<0.05),癌组织中BMAL1蛋白表达阳性为胃癌患者ESD后复发的独立保护因素(P<0.05).结论 早期胃癌组织中PDE1B蛋白表达阳性率升高,BMAL1蛋白表达阳性率降低,与ESD后复发均密切相关.

Objective To investigate the relationship between phosphodiesterase 1B(PDE1B),brain and muscle aryl hydrocarbon receptor nuclear transporter like 1(BMAL1)protein expression and recurrence of early gastric cancer after endoscopic submucosal dissection(ESD).Methods A total of 155 patients with gas-tric cancer who underwent ESD in this hospital from January 2018 to December 2021 were selected as the re-search objects.Followed up for 3 yesrs all gastric cancer patients were divided into recurrence group and non-recurrence group according to whether they had recurrence after ESD.The expression of PDE1B and BMAL1 protein in early gastric cancer tissues and adjacent tissues was detected by immunohistochemistry.The recur-rence-free survival rates of patients with positive and negative PDE1B and BMAL1 protein expression after ESD were compared by K-M survival curve,and Log-rank χ2 test was performed.Multivariate Cox regression analysis was used to analyze the influencing factors of recurrence after ESD in gastric cancer patients.Results The positive expression rate of PDE1B protein in gastric cancer tissues was higher than that in adja-cent tissues,and the positive expression rate of BMAL1 protein was lower than that in adjacent tissues,and the differences were statistically significant(P<0.05).After 3 years of follow-up,45(29.03%)of 155 gas-tric cancer patients relapsed after ESD.The positive expression rate of cancer tissue PDE1B protein in the re-currence group was higher than that in the non-recurrence group,and the positive expression rate of BMAL1 protein in the recurrence group was lower than that in the non-recurrence group,and the differences were sta-tistically significant(P<0.05).The results of K-M survival curve analysis showed that the recurrence-free survival curve of PDE1B protein positive patients after ESD was lower than that of PDE1B protein negative patients(Log-rank χ2=23.290,P<0.001).The recurrence-free survival curve of patients with positive BMAL1 protein expression after ESD was higher than that of patients with negative BMAL1 protein expres-sion(Log-rank χ2=7.676,P=0.006).Multivariate Cox regression analysis showed that poor differentiation and positive expression of cancer tissue PDE1B protein were independent risk factors for recurrence of gastric cancer patients after ESD(P<0.05),and positive expression of cancer tissue BMAL1 protein was an inde-pendent protective factor for recurrence of early gastric cancer patients after ESD(P<0.05).Conclusion The posi-tive expression rate of PDE1B protein in gastric cancer tissues is increased,and the positive expression rate of BMAL1 protein is decreased,which is closely related to the recurrence of gastric cancer after ESD.
