基于癌毒-态靶理论探讨多发性骨髓瘤肾病中西医整合辨治策略OA

Based on the theory of cancer toxins,proposed by national master of traditional Chinese medicine(TCM)Zhou Zhongying,and the state-target theory,advocated by Academician Tong Xiaolin,Ni Haiwen uses the cancer toxins-state-targets theory to guide the clinical diagnosis and treatment of multiple myeloma(MM)with integration of the TCM and Western medicine.MM is categorized into five distinct stages:the symptom-concealed stage,the symptom-active stage,the target organ damage stage,the disease-stable stage and the disease-terminal stage.MM nephropathy occurs during the target organ damage stage.The core pathogenesis throughout the disease progression involves kidney deficiency complicated by cancer toxins,along with the persistent entanglement of dampness and stasis.Clinically,therapeutic strategies focus on reinforcing kidney function to eliminate toxins and resolving dampness to dissipate stasis.The MM-targeted formula,Xuanbi Xiaoliu Formula,is combined with Serissa japonica and Pteris multifida to enhance detoxification and promote the excretion of turbid pathogens,thereby forming the MM nephropathy-targeted formula known as Qingli Xuanbi Xiaoliu Formula.In cases involving complex syndromes of phlegm-turbidity and stasis-toxins,specific therapeutic interventions such as resolving phlegm to remove turbidity and expelling stasis to detoxify are applied.For specific clinical manifestations such as lumbar soreness and edema,symptom-targeted herbs are incorporated;for the treatment of elevated creatinine levels and other abnormal laboratory indicators,biomarker-targeted herbs are selected,so that the disease and syndrome can be treated,the root and manifestation can be addressed.Thereby,an integrative TCM-Western diagnostic and treatment strategy strategy for managing multiple myeloma nephropathy is formed under the guidance of cancer toxins-state-targets theory.This approach also includes the development of targeted herbal formulas and pharmacological interventions aimed at optimizing clinical outcomes.
