Causal Association Between Tongue-Coating Microbiome and Cardiovascular Diseases Based on the Theory of"The Tongue is the Orifice to the Heart":A Two-Sample Mendelian Randomization Study

目的:运用双样本孟德尔随机化(MR)方法,系统探究舌苔菌群与心血管疾病(CVD)的因果关系.方法:基于欧洲人群心血管疾病全基因组关联研究(GWAS)数据(177 923个样本,共获得9 587 836个SNPs)及舌苔菌群数据(n=2 017),筛选强相关遗传工具变量(P<1×10-5,F>10),以逆方差加权法(IVW)为主分析,辅以敏感性检验(异质性、多效性、留一法).结果:共发现140种舌苔菌群与CVD显著相关(IVW P<0.05),其中,67种菌群的丰度增加与疾病风险的增加相关(OR值1.01~1.04),而73种菌群的丰度增加则呈现出对疾病的保护作用(OR值0.97~0.99),敏感性分析显示结果稳健(MR-Egger截距P>0.05,无反向因果).结论:通过双样本MR分析,首次从遗传学角度揭示了舌苔菌群与心血管疾病之间的因果关联,为"心在窍为舌"的中医理论提供了现代科学依据,丰富了中医舌诊的理论内涵,并为中医药通过调节舌苔菌群防治心血管疾病提供了新的依据.

Objective:To systematically investigate the causal relationship between tongue-coating microbiota and cardiovascular disease(CVD),using a two-sample Mendelian randomization(MR)approach.Methods:Genetic instruments strongly associated with tongue-coating microbiota were selected(P<1×10-5,F>10)from European-descent Genome-Wide Association Study(GWAS)summary statistics of CVD(total sample:177,923;9,587,836 SNPs)and tongue-coating microbiota(n=2,017).The inverse-variance weighted(IVW)method served as the primary analysis,supplemented by sensitivity analyses(heterogeneity testing,pleiotropy assessment via MR-Egger intercept,and leave-one-out validation).Results:A total of 140,140 tongue-coating microbiota were identified to have significant associations with CVD(IVW P<0.05).Among these,increased abundance of 67 microbiota was associated with elevated CVD risk(OR range:1.01-1.04),while increased abundance of 73 microbiota demonstrated protective effects against CVD(OR range:0.97-0.99).Sensitivity analyses confirmed the robustness of these associations(MR-Egger intercept P>0.05,no evidence of reverse causality).Conclusion:This two-sample MR study provides the first genetic evidence supporting a causal link between tongue-coating microbiota and CVD,and validates the traditional Chinese medicine(TCM)theory that"the tongue is the orifice to the heart"(Xin Zai Qiao Wei She).Meanwhile,it enriches the theoretical foundation of TCM tongue diagnosis.Furthermore,this work offers novel scientific rationale for TCM strategies to targeting tongue-coating microbiota modulation to prevent and treat CVD.
