目的 感音神经性听力损失(sensorineural hearing loss,SNHL)是全球范围内较为常见的听力障碍,与多种因素相关.近年来,部分研究表明,体力活动(physical activity,PA)可能对SNHL具有保护作用,但其因果关系尚未明确.本研究采用孟德尔随机化(Mendelian randomization,MR)分析,探讨PA与SNHL之间的潜在因果关系,以提供更有力的因果推断.方法 本研究基于已发表的全基因组关联研究(genome-wide association studies,GWAS)汇总数据,采用双样本MR分析.数据来自公开的MRC IEU GWAS数据库.选择与PA和SNHL显著相关的单核苷酸多态性作为工具变量,在分析中,选择了两个PA表型:每周进行10 min以上中等强度PA的天数(记为:"MPA 10+min",GWAS ID:ukb-a-508,样本量为321 309)和每周进行10 min以上高强度PA的天数(记为:"VPA 10+min",GWAS ID:ukb-b-151,样本量为440 512).为避免暴露人群和结局人群的重叠,SNHL数据集来自FinnGen(finn-b-H8_HL_SEN_NAS),包括15 952例病例和196 592个对照组.使用逆方差加权法(inverse-variance weighted,IVW)、MR Egger方法和加权中位数(weighted median,WM)方法进行MR分析.通过MR-Egger回归和MR-PRESSO方法检验水平多效性,并使用Cochran's Q检验评估异质性.此外,进行留一法分析以验证结局的稳健性和可靠性.结果 IVW分析显示PA与SNHL相对风险呈负相关("MPA 10+min":IVW:OR=0.689,P=0.013,95%CI=0.514~0.924;"VPA 10+min":OR=0.625,P=0.001,95%CI=0.468~0.835).WM法与IVW法结局一致.敏感性分析未发现异质性或水平多效性证据.留一法证实了结局的稳健性,表明没有单个工具变量影响研究结局.结论 本研究基于MR分析提供了PA可能降低SNHL风险的因果证据.这一发现为PA在听力保护方面的潜在作用提供了支持,但在未来仍需进一步研究验证并探索其潜在机制.

Objective Sensorineural hearing loss(SNHL)is a common auditory disorder worldwide and may be associated with multiple factors.Recent studies suggest that physical activity(PA)may have a protective effect against SNHL,but the causal relationship remains unclear.This study employs Mendelian randomization(MR)analysis to investigate the potential causal relationship between PA and SNHL.Methods A two-sample MR analysis was conducted based on summary statistics from published genome-wide association studies(GWAS).Data were obtained from the publicly available MRC IEU GWAS database.Single nucleotide polymorphisms(SNPs)significantly associated with PA and SNHL were selected as instrumental variables.Two PA phenotypes were included in the analysis:the number of days per week engaging in at least 10 minutes of moderate-intensity physical activity(MPA 10+min)(GWAS ID:ukb-a-508,sample size=321 309)and the number of days per week engaging in at least 10 minutes of vigorous-intensity physical activity(VPA 10+min)(GWAS ID:ukb-b-151,sample size=440 512).To avoid sample overlap between exposure and outcome groups,SNHL data were obtained from the FinnGen consortium(finn-b-H8_HL_SEN_NAS),comprising 15 952 cases and 196 592 controls.MR analyses were performed using the inverse-variance weighted(IVW)method,MR-Egger regression,and the weighted median(WM)approach.Horizontal pleiotropy was assessed using MR-Egger regression and MR-PRESSO,while heterogeneity was evaluated using Cochran's Q test.Additionally,leave-one-out sensitivity analysis was conducted to ensure the robustness and reliability of the results.Results The IVW analysis demonstrated a negative association between PA and the relative risk of SNHL("MPA 10+min":IVW:OR=0.689,P=0.013,95%CI=0.514~0.924;"VPA 10+min":OR=0.625,P=0.001,95%CI=0.468~0.835).The WM method yielded results consistent with those of the IVW method.Sensitivity analyses indicated no evidence of heterogeneity or horizontal pleiotropy.The leave-one-out analysis confirmed that the findings were robust and not influenced by any individual instrumental variable.Conclusions This MR study provides causal evidence suggesting that PA may reduce the risk of SNHL.These findings support the potential role of PA in hearing protection.However,further research is needed to validate these results and explore underlying biological mechanisms.
