眼震潜伏期对水平管石BPPV患侧判定的价值OA
The value of nystagmus latency in determination of affected side in horizontal semicircular canal BPPV
目的 探讨眼震潜伏期(latency of nystagmus,LON)对水平管石良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)患侧判定的价值.方法 纳入2023年1 月至2024 年9 月北京大学人民医院青岛医院耳鼻咽喉头颈外科眩晕门诊收治的91例水平管石BPPV患者,先行俯屈仰卧试验(bow and supine test,BST),再行180°滚转试验(roll test,RT),观察眼震情况.若RT两侧眼震强度比约>3∶2,则根据RT结果复位;若RT不能判定患侧,则根据BST结果复位;若前两种方法均判断不出患侧,则根据行RT时诱发的LON结果进行复位,LON较长侧(至少长1 s)为患侧.复位后如验证失败,则换反方向复位.统计BST、RT及LON判断水平管石BPPV患侧的敏感性及正确率.结果 入组的91例患者,71例(78%,71/91)BST诱发出眼震;75例(82.4%,75/91)RT可以判断出患侧;62例(68.1%,62/91)LON可以判断出患侧.BST正确率为72.5%(66/91),RT正确率为78%(71/91),LON正确率为67%(61/91),3种方法正确率无统计学差异(P>0.05).ROC曲线说明3种方法判定患侧均具有高准确率,3组曲线下面积无统计学差异.91例患者全部复位成功,其中初次复位错误有5例患者,有3例转变为嵴顶耳石症,后按反方向复位均成功.结论 行180°滚转试验观察眼震潜伏期能够显著提高水平管石BPPV患侧判别的成功率,不会给患者增加负担,可作为辅助判断水平管石BPPV患侧的客观指标,值得临床推广.
Objective To investigate the value of nystagmus latency in determining the affected side In horizontal semicircular canal benign paroxysmal positional vertigo(BPPV).Methods Patients with horizontal semicircular canal BPPV(n=91)admitted to the Vertigo Clinic of the Department of Otolaryngology,Head and Neck Surgery,Peking University People's Hospital in Qingdao from January 2023 to September 2024 were subjected to bow and supine test(BST),followed by 180°roll test(RT),while observed for nystagmus.Repositioning therapies were performed based on RT results if the side to side ratio of nystagmus intensity was greater than 3:2,or based on BST results if RT failed to indicate the affected side.When the affected side could not be determined by either RT or BST,the longer latency of nystagmus(LON)in RT(at least 1 s)was used to determine the affected side.If failing verification after repositioning,the maneuvers were repeated in the opposite direction.The sensitivity and rate of correct side determination for BST,RT and LON were analyzed.Results Of the 91 patients,nystagmus was induced in 71(78%)by BST.Side determination was successful in 75(82.4%)cases by RT and in 62 cases(68.1%)by LON.The rate of correction side determination was 72.5%(66/91)for BST,78%(71/91)for RT,and 67%(61/91)for LON,respectively(P>0.05).ROC indicated high accuracy in determining the affected side for all three methods.Repositioned was successful in all 91 patients,including 5 whose initial repositioning was erroneous resulting in cupulotolithiasis in 3 patients and in whom repositioning in the opposite direction was successfully.Conclusions Observing nystagmus latency in the 180°RT can significantly improve the rate of success in identification of the affected side in horizontal semicircular canal BPPV without increasing the burden on the patient,and is worthy of clinical adoption.
