PDGF-BB/PDGFR-β信号通路与糖尿病视网膜病变病情进展及预后的相关性OA
Correlation between the platelet-derived growth factor-BB/platelet-derived growth factor receptor-β signaling pathway and the progression and prognosis of diabetic retinopathy
目的 分析糖尿病视网膜病变(DR)患者血小板源性生长因子-BB(PDGF-BB)、血小板源性生长因子受体-β(PDGFR-β)水平,探究其与病情进展及预后的相关性.方法选取山东中医药大学附属医院 2022 年 5 月至 2025 年 5 月DR患者 116 例(116 眼)为DR组,另选取同期无DR的单纯糖尿病(DM)患者116 例(116 眼)为非DR(NDR)组.比较DR组与NDR组、DR组不同病情程度患者房水和血清中PDGF-BB、PDGFR-β水平,分析PDGF-BB、PDGFR-β与DR病情的相关性;DR组随访1 年,比较不同预后患者临床资料以及房水、血清PDGF-BB、PDGFR-β水平,Logistic分析各指标与预后关系,绘制受试者工作特征(ROC)曲线分析房水和血清中PDGF-BB、PDGFR-β水平对患者预后的预测价值.结果 DR组患者房水、血清 PDGF-BB、PDGFR-β水平均高于 NDR组(均为 P<0.05).DR组增生型糖尿病视网膜病变患者房水、血清PDGF-BB、PDGFR-β水平均高于非增生型糖尿病视网膜病变患者(均为P<0.05).房水、血清PDGF-BB、PDGFR-β水平与DR病情进展均呈正相关(均为P<0.05).视力残疾患者DM病程长于无视力残疾患者,糖化血红蛋白、房水、血清PDGF-BB、PDGFR-β水平均高于无视力残疾患者(均为P<0.05).房水、血清PDGF-BB、PDGFR-β水平均与预后独立相关(均为P<0.05).房水、血清PDGF-BB、PDGFR-β水平预测DR患者视力残疾的AUC为0.811、0.812、0.754、0.759.结论 DR患者房水、血清中PDGF-BB、PDGFR-β高表达,且与患者病情进展、预后密切相关,且房水、血清中PDGF-BB、PDGFR-β对患者预后具有较高的预测价值.
Objective To analyze the levels of platelet-derived growth factor-BB(PDGF-BB)and platelet-derived growth factor receptor-β(PDGFR-β)in patients with diabetic retinopathy(DR)and investigate their correlation with the progression and prognosis of DR.Methods A total of 116 patients(116 eyes)with DR in the Affiliated Hospital of Shan-dong University of Traditional Chinese Medicine from May 2022 to May 2025 were selected as the DR group,and another 116 patients(116 eyes)with diabetes mellitus(DM)without DR during the same period were selected as the non-DR(NDR)group.The levels of PDGF-BB and PDGFR-β in aqueous humor and serum were compared between the DR group and the NDR group,as well as among the patients with different disease severities in the DR group,and the correlation between PDGF-BB/PDGFR-β levels and DR progression was analyzed.The DR group was followed up for 1 year.The clinical data,as well as the levels of PDGF-BB and PDGFR-β in aqueous humor and serum,were compared between patients with different prognostic outcomes.Logistic analysis was used to analyze the relationship between each index and prognosis.The receiver operating characteristic(ROC)curves were drawn to analyze the predictive value of PDGF-BB and PDGFR-β levels in aque-ous humor and serum for the prognosis of patients.Results The levels of PDGF-BB and PDGFR-β in aqueous humor and serum of patients in the DR Group were higher than those in the NDR group(both P<0.05).The levels of PDGF-BB and PDGFR-β in aqueous humor and serum of patients with proliferative DR in the DR group were higher than those of patients with non-proliferative DR(both P<0.05).The levels of PDGF-BB and PDGFR-β in aqueous humor and serum were both positively correlated with the progression of DR(both P<0.05).The course of DM in patients with visual disability was longer than that in patients without visual disability,and their levels of HbAlc,as well as PDGF-BB and PDGFR-β in aqueous humor and serum,were all higher than those of patients without visual disability(all P<0.05).The levels of PDGF-BB and PDGFR-β in aqueous humor and serum were independently correlated with prognosis(both P<0.05).The areas under the curve(AUC)of PDGF-BB and PDGFR-β levels in aqueous humor and serum for predicting visual impairment in DR patients were 0.811,0.812,0.754,and 0.759.Conclusion PDGF-BB and PDGFR-β are highly expressed in the serum and aqueous humor of DR patients and are closely correlated with the progression and prognosis of patients.PDGF-BB and PDGFR-β in serum and aqueous humor have high predictive value for the prognosis of patients.
