Efficacy of defocus incorporated multiple segments spectacle lenses in con-trolling myopia progression in adolescents and changes in astigmatism

目的 探讨青少年近视患者配戴多区正向光学离焦(DIMS)镜片 1 年期的近视控制效果及散光的变化.方法 本研究随机选取 2023 年 1 月至 2024 年 1 月于苏州大学理想眼科医院验配DIMS镜片的青少年近视者作为研究对象,根据受试者是否具有需要矫正的散光,将其分为两组:无散光组 33 例(33 眼),散光度>-0.50 D;散光组 34 例(34眼),散光度-2.50~-0.50 D.于戴镜前(基线)及戴镜后 3 个月、6 个月、1 年分别测量并比较各组受试者眼轴长度、等效球镜度(SER)、散光度、平坦角膜曲率(Kf)、陡峭角膜曲率(Ks)及平均角膜曲率(Km).采用重复测量方差分析评估各参数随时间变化的组内差异,若存在统计学意义,进一步使用Bonferroni法进行事后两两比较.此外,以戴镜前数据作为基线,运用Bland-Altman法评估上述参数在1 年随访时与基线的一致性.结果 戴镜前散光组受试者Km与Ks均大于无散光组(均为P<0.05).在戴镜后3 个月、6 个月及1年三个随访时间点,除散光度、Km及Ks外,两组受试者间在其余参数上的差异均无统计学意义(均为P>0.05).戴镜后1 年,散光组受试者散光度小于无散光组,Km及Ks均大于无散光组(均为P<0.05).经 Bonferroni 法两两比较发现,两组受试者眼轴长度、SER及无散光组受试者散光度在戴镜前与戴镜后 1 年之间的差异均有统计学意义(均为P<0.008).戴镜后1 年与戴镜前相比,眼轴长度变化≤0.20 mm的比例:总体为32.54%,无散光组为39.39%,散光组为26.74%.眼轴长度变化≤0.30 mm的比例:总体为55.22%,无散光组为60.61%,散光组为 50.00%.SER变化≥-0.50 D的比例:总体为 53.73%,无散光组为57.58%,散光组为50.00%.散光度变化≥-0.50 D的比例:总体为95.52%,无散光组为96.88%,散光组为 97.06%.结论 配戴DIMS镜片能有效控制近视青少年的眼轴及SER增长,戴DIMS镜片后患者的散光变化存在统计学意义但无临床意义.

Objective To investigate the myopia control efficacy and changes in astigmatism following one-year wearing of defocus incorporated multiple segments(DIMS)lenses in adolescents with myopia.Methods Adolescent my-opic patients fitted with DIMS lenses in Lixiang Eye Hospital of Soochow University from January 2023 to January 2024 were randomly selected in this study.According to whether the subjects had astigmatism that needed to be corrected,they were divided into two groups:non-astigmatism group(33 patients,33 eyes),with astigmatism>-0.50 D;astigmatism group(34 patients,34 eyes),with astigmatism ranging from-2.50 D to-0.50 D.The axial length(AL),spherical equivalent refrac-tion(SER),astigmatism,flat keratometry(Kf),steep keratometry(Ks),and mean keratometry(Km)of the aforemen-tioned myopic adolescents were measured and compared before wearing glasses(baseline)and at 3 months,6 months,and 1 year after wearing glasses.Repeated measures ANOVA was used to evaluate the intra-group differences in each parameter over time,and the Bonferroni method was further used for post hoc pairwise comparison if there was statistical signifi-cance.In addition,the Bland-Altman method was used to evaluate the consistency of the above parameters with the baseline data(before wearing glasses)at 1 year follow-up.Results The Km and Ks at baseline in the astigmatism group were greater than those in the non-astigmatism group(both P<0.05).Except astigmatism,Km and Ks,there was no significant difference in the other parameters between the two groups at 3 months,6 months,and 1 year after wearing glasses(all P>0.05).At 1 year after wearing glasses,patients in the astigmatism group had smaller astigmatism but larger Km and Ks than the non-astigmatism group(all P<0.05).Pairwise comparison using the Bonferroni method revealed that there were statis-tically significant differences in AL and SER in both groups,as well as astigmatism in the non-astigmatism group,between before and after one year of wearing glasses(all P<0.008).Compared with the baseline data,the proportion of AL change≤0.2 mm at 1 year after wearing glasses was 32.54%overall,39.39%in the non-astigmatism group,and 26.74%in the astigmatism group;the proportion of AL change≤0.3 mm was 55.22%overall,60.61%in the non-astigmatism group,and 50.00%in the astigmatism group;the proportion of SER change≥-0.50 D was53.73%overall,57.58%in the non-astig-matism group,and 50.00%in the astigmatism group;the proportion of astigmatism change≥-0.50 D was95.52%overall,96.88%in the non-astigmatism group,and 97.06%in the astigmatism group.Conclusion Wearing DIMS lenses can effectively control the growth of AL and SER in myopic adolescents.The astigmatism changes after wearing DIMS lenses are statistically significant but have no clinical significance.
