目的:探究电针(EA)足三里对肝叶部分切除术后小鼠免疫功能及炎症反应的影响.方法:按计算机生成的随机数字表法将C57 BL/6 小鼠随机分成空白组、模型组和电针组.模型组和电针组在全身麻醉下制备 70%肝叶切除手术动物模型.电针组分别于术前 1d、术后第 1、2、3 天于双侧足三里(ST36)处行EA治疗.空白组和模型组不予 EA治疗.在术后的第 4 天,各组小鼠取血和脾脏,进行细胞因子(IL-4、IL-10)和淋巴细胞亚群(CD4+T细胞、CD8+T细胞)的测定,以评估 EA对术后小鼠免疫功能和炎症指标的影响.其余各组小鼠给予绿脓杆菌滴鼻,6 h后各组随机选取半数的小鼠进行取材(肺),涂板进行细菌培养,并统计菌落数量.剩余各组小鼠在接种后 5 d观察各组小鼠的死亡率,以评估小鼠清除继发感染细菌能力.结果:与空白组比较,模型组小鼠在术后第 4 天血清 IL-4和IL-10 水平明显上升,脾脏中CD4+T细胞比例明显减少,肺组织菌落数显著增加和感染后生存率显著降低(P<0.05).与模型组小鼠相比,电针组小鼠血清 IL-4 和 IL-10 水平明显下降,脾脏中 CD4+T细胞比例明显增加,肺组织菌落数明显减少和感染后生存率显著上升(P<0.05).结论:EA足三里可以减轻小鼠肝叶部分切除术后的免疫抑制,并通过降低术后的炎症反应来发挥保护作用.

Objective:To investigate the effects of electroacupuncture(EA)at Zusanli(ST36)on immune func-tion and inflammatory response in mice undergoing partial hepatectomy.Methods:C57 BL/6 mice were randomly allo-cated into a blank group,a model group,and an EA group using a computer-generated random number table.The model and EA groups underwent 70%partial hepatectomy under general anaesthesia to establish the surgical trauma model.The EA group received electroacupuncture at bilateral ST36 one day prior to surgery and on postoperative days 1,2 and 3.The blank and model groups did not receive EA treatment.On postoperative day 4,blood and spleen samples were collected from each group to measure cytokine levels(IL-4,IL-10)and lymphocyte subsets(CD4+T cells,CD8+T cells)to assess the impact of EA on postoperative immune function and inflammatory markers.The re-maining mice in each group were subjected to Pseudomonas aeruginosa nasal inoculation.Six hours later,half of the mice in each group were randomly selected for lung tissue collection,bacterial culture plating,and colony counting.The survival rates of the remaining mice were monitored for five days post-inoculation to evaluate their ability to clear secondary bacterial infections.Results:Compared with the blank group,the model group exhibited significantly elevated IL-4 and IL-10 levels,a marked reduction in splenic CD4+T cell proportion,a significant increase in lung bacterial colonies,and a notably lower post-infection survival rate on postoperative day 4(P<0.05).In contrast,the EA group demonstrated significantly lower serum anti-inflammatory cytokine(IL-4,IL-10)levels,an increased proportion of splenic CD4+T cells,fewer lung bacterial colonies,and improved post-infection survival rates compared to the model group(P<0.05).Conclusion:Electroacupuncture at ST36(Zusanli)alleviated immunosuppression in mice following partial hepatectomy and exerted a protective effect by reducing the postoperative inflammatory response.
