目的 研究PD-L1在表皮生长因子(epidermal growth factor receptor,EGFR)野生型非小细胞肺癌(non-small cell lung cancer,NSCLC)中CD44调控作用下的不同表达,并揭示过程中可能的分子机制,为EGFR野生型NSCLC靶向和免疫治疗提供新的靶点.方法 ① 采用生物信息学方法,结合TCGA数据库系统进行数据检索和分析,探索EGFR野生型NSCLC中CD44与PD-L1的表达相关性,以及两者共高表达与肺癌患者的生存率分析.② 体外培养人EGFR野生型NSCLC细胞系(H1299、H460、A549),利用流式细胞术、qPCR、western blot检测CD44经敲低或过表达处理后对EGFR野生型NSCLC表面标志物PD-L1的阳性细胞群体比例、mRNA及蛋白表达水平的影响.③ 采用GraphPad Prism软件(v9)进行统计分析和创建图形,定量数据以均值±标准差表示,组间差异采用非配对t检验进行统计学分析.结果 生信分析结果表明,EGFR野生型NSCLC中CD44表达与PD-L1表达水平呈显著正相关(肺腺癌:P<0.05,R=0.54;肺鳞癌:P<0.05,R=0.19),且两者高表达与肺癌病人低生存显著相关(HR=0.65,95%CI:1.57~2.57,P=0.023).在H1299、H460及A549细胞中,CD44敲低处理之后细胞表面标志物PD-L1的阳性细胞群体比例、mRNA及蛋白表达水平显著降低(P<0.05).基因CD44经过表达处理之后,H1299、H460细胞表面标志物PD-L1的mRNA及蛋白表达水平上升(P<0.05).结论 EGFR野生型NSCLC细胞中CD44对PD-L1具有明显的正向调控作用,且两者共高表达与肺癌的预后不良相关.

Objective This project primarily investigated the regulation of CD44 on PD-L1 in EGFR wild-type Non-small cell lung cancer(NSCLC),and revealed the potential molecular mechanism of the process,aiming to identify a novel target for targeted therapy and cancer immunotherapy of EGFR wild-type NSCLC.Methods 1.Bioinformatics analysis was employed in conjunction with TCGA database for data retrieval and analysis to investigate the association between CD44 and PD-L1 expression in EGFR wild-type NSCLC,as well as the survival rate analysis of patients with high co-expression of both.2.Human EGFR wild-type NSCLC cell lines(H1299,H460,A549)were cultured in vitro.Flow cytometry,qPCR and western-blot were utilized to assess the impact of CD44 knockdown or overexpression on the positive cell population ratio,mRNA and protein expression levels of PD-L1 in EGFR wild-type NSCLC cells.3.GraphPad Prism software(v9)was used for statistical analysis and graph creation.Quantitative data are presented as mean±standard deviation(Mean±SD),and intergroup differences were analyzed by unpaired t-test.Results Bioinformatics analy-sis showed a significant positive correlation between CD44 expression and PD-L1 expression levels in EGFR wild-type NSCLC(Lung Adenocarcinoma:P<0.05,R=0.54;Lung squamous cell carcinoma:P<0.05,R=0.19).Moreover,the high expression level of CD44 and PD-L1 is associated with decreased overall survival of NSCLC patients when compared with CD44 and PD-L1 negative patients(HR=0.65,95%CI:1.57~2.57,P=0.023).In H1299,H460 and A549 cells,the proportion of positive cell populations,mRNA and protein expression levels of PD-L1 were significantly decreased after CD44 knockdown(P<0.05).After CD44 overexpres-sion,the mRNA and protein expression levels of PD-L1 in H1299 and H460 cells were increased(P<0.05).Conclusion CD44 in EGFR wild-type NSCLC cells has a positive regulatory effect on PD-L1,and the co-high expression of both is associated with a poor prognosis of lung cancer.
