Efficacy Observation of Supplemented Honey-Fried Licorice Decoction in Treating 45 Cases of Paroxysmal Atrial Fibrillation with Qi-Yin Deficiency Syndrome

目的:观察炙甘草汤加味治疗阵发性房颤气阴两虚证的疗效及安全性.方法:选取 2022 年 1 月至 2024 年 10 月河南省荣军医院收治的阵发性房颤患者90 例,随机分为对照组和观察组各45 例,对照组采用小剂量胺碘酮治疗,观察组在对照组治疗基础上采用炙甘草汤加味治疗,两组均连续治疗3 个月.比较两组患者的临床疗效及治疗前后心率变异性指标、肌酸激酶(creatine kinase,CK)、α-羟丁酸脱氢酶(α-hydroxybutyrate dehydrogenase,α-HBDH)、丙二醛(malondialdehyde,MDA)、基质金属蛋白酶-2(matrix metallo proteinases-2,MMP-2)水平.记录两组患者治疗期间发生的腹痛、腹泻、恶心等不良反应发生率及心电图表现差异.结果:对照组有效率为73.33%,观察组有效率为91.11%,两组有效率比较,差异有统计学意义(P<0.05).两组患者治疗后,连续R-R间期标准差(standard deviation of NN intervals,SDNN)、5 minR-R间期均值的标准差(standard deviation of all mean 5 minute RP intervals,SDANN)较治疗前均升高(P<0.05),且观察组高于对照组(P<0.05),全程相邻正常窦性心搏间期之差的均方根值(the root mean square successive differences,rMMSD)、相邻正常窦性心搏间期差值50 ms 的百分数(root mean square of successive differences,PNN50)治疗前后差异无统计学意义(P>0.05).两组患者治疗后MMP-2、CK、MDA、α-HBDH 水平较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05).两组患者治疗后最大P波时限、P波离散度均降低,且观察组低于对照组(P<0.05).对照组发生腹痛 2 例,不良反应发生率为 4.44%,观察组发生腹泻1 例、恶心2 例,不良反应发生率为6.67%,两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:炙甘草汤加味联合小剂量胺碘酮治疗阵发性房颤可改善患者心率变异性,提高疗效,且无明显不良反应.

Objective:To observe the efficacy and safety of Supplemented Honey-Fried Licorice Decoction in treating paroxysmal atrial fi-brillation(PAF)with qi-yin deficiency syndrome.Methods:A total of 90 patients with PAF admitted to the hospital from January 2022 to October 2024 were randomly divided into the control group and the observation group,with 50 cases in each group.The control group received low-dose Amiodarone,while the observation group received Supplemented Honey-Fried Licorice Decoction in addition to the control group's treatment.Both groups were treated for 3 months.Then the efficacy,heart rate variability(HRV)indicators,creatine ki-nase(CK),α-hydroxybutyrate dehydrogenase(α-HBDH),malondialdehyde(MDA),and matrix metalloproteinase-2(MMP-2)levels were compared between the two groups.The incidence of adverse reactions,such as abdominal pain,diarrhea,nausea,etc.and electro-cardiographic findings were recorded.Results:The effective rate was 73.33%in the control group and 91.11%in the observation group,with a statistically significant difference(P<0.05).After treatment,SDNN and SDANN increased in the two groups(P<0.05),and the above indicators of the observation group were higher than those of the control group(P<0.05).No significant differ-ences were observed in rMMSD or PNN50 before and after treatment(P>0.05).Levels of MMP-2,CK,MDA,and α-HBDH decreased in the two groups(P<0.05),and the above indicators of the observation group were lower than those of the control group(P<0.05).Maximum P-wave duration and P-wave dispersion decreased in the two groups(P<0.05),with greater reductions in the observation group(P<0.05).In the control group,abdominal pain occurred in2 cases,with an incidence of adverse reactions of 4.44%.In the observation group,diarrhea occurred in 1 case and nausea in 2 cases,with an incidence of adverse reactions of 6.67%.When comparing the incidence of adverse reactions between the two groups,there was no statistically significant difference(P>0.05).Conclusion:Sup-plemented Honey-Fried Licorice Decoction combined with low-dose Amiodarone can improve HRV and efficacy in treating PAF with qi-yin deficiency syndrome,without significant adverse reactions.
