基于代谢重编程探讨糖尿病肾脏病气虚血瘀证实质OA
Pathogenesis Essence of Diabetic Kidney Disease with Qi deficiency and Blood Stasis Syndrome Based on Metabolic Reprogramming
糖尿病肾脏病(diabetic kidney disease,DKD)是糖尿病患者重要的微血管并发症,可能导致肾功能衰竭.代谢重编程以物质与能量流向失衡为核心,其特征包括线粒体功能障碍、糖酵解增加和脂质代谢异常等.气虚血瘀是DKD的关键病理机制,代谢重编程贯穿于DKD病理演变的整个过程,在其发展中起到推动作用,而DKD的存在也加速了代谢重编程的过程.益气活血化瘀法为DKD的重要治法,通过标本同治,改善线粒体功能,调节代谢平衡,抑制氧化应激,改善肾功能.重塑代谢重编程有助于恢复正常的代谢秩序,从而减缓或逆转DKD的进展.
Diabetic kidney disease(DKD)is an important microvascular complication in patients with diabetes and may lead to renal failure.Metabolic reprogramming,characterized by a core imbalance of material and energy flow,includes mitochondrial dysfunction,in-creased glycolysis,and lipid metabolism abnormalities.Qi deficiency and blood stasis are key pathological mechanisms in DKD.Meta-bolic reprogramming runs throughout the entire pathological progression of DKD,playing a promoting role in its development,while the presence of DKD also accelerates the process of metabolic reprogramming.The method of tonifying Qi and activating blood to resolve stasis is an important treatment for DKD.By treating both the root and the branch,it improves mitochondrial function,regulates metabol-ic balance,inhibits oxidative stress,and enhances renal function.Remodeling metabolic reprogramming helps restore normal metabolic order,thereby slowing down or reversing the progression of DKD.
