目的:研究径向超声小探头技术联合Ras相关区域家族1A(RASSF1A)基因、矮小同源盒基因(SHOX2)甲基化检测在肺外周实性/亚实性结节中的诊断价值.方法:选取2021年7月至2023年 10月我院收治的 302例外周肺结节患者,经支气管镜行径向超声小探头技术引导肺活检,并经实时荧光PCR检测肺泡灌洗液中RASSF1A、SHOX2 基因的甲基化状态.结果:与径向超声小探头、RASSF1A、SHOX2 基因甲基化单独诊断比较,联合诊断灵敏度(99.45%)、特异度(100.00%)、准确度(99.67%)较高(P<0.05);径向超声小探头+RASSF1A、SHOX2基因甲基化相对于细胞学可将恶性肺结节诊断灵敏度从 50.82%提高至 99.45%,特异度从 97.48%提高至100.00%;径向超声小探头技术联合RASSF1A、SHOX2 基因甲基化检测的阳性率在细胞学发现癌细胞组为 100.00%,结果未明组为 100.00%,未发现癌细胞组为 43.10%;径向超声小探头+RASSF1A、SHOX2基因甲基化对亚实性恶性肺结节检出率高于径向超声小探头(P<0.05).结论:径向超声小探头技术联合RASSF1A、SHOX2基因甲基化检测可有效提高肺外周实性/亚实性结节检出率,为临床早期筛查提供参考依据.

Objective:To investigate the diagnostic value of combining radial ultrasound small probe technology with Ras-related region family 1A(RASSF1A)gene and short homeobox gene 2(SHOX2)methylation detection in the diagnosis of peripheral solid/subsolid nodules in the lung.Methods:A total of 302 patients with peripheral pulmonary nodules admitted to our hospital from July 2021 to October 2023 underwent lung biopsy guided by the radial ultrasound small probe technique via bronchoscopy,and the methylation status of RASSF1A and SHOX2 genes in bronchoalveolar lavage fluid was detected by real-time fluorescence PCR.Results:Compared with radial ultrasound probe,RASSF1A,SHOX2 gene methylation alone,the combined diagnostic sensitivity of 99.45%,specificity of 100.00%,accuracy of 99.67%were higher(P<0.05).Compared with cytology,the sensitivity and specificity of the diagnosis of malignant pulmonary nodules increased from 50.82%to 99.45%and from 97.48%to 100.00%with the addition of RASSF1A and SHOX2 gene methylation.The positive rate of methylation of RASSF1A and SHOX2 genes was 100.00%in the cytological detection group,100.00%in the unknown group and 43.10%in the undetected group,which could effectively improve the detection rate.The detection rate of RASSF1A and SHOX2 gene methylation on subsolid malignant pulmonary nodules was higher than that of radial ultrasound probe(P<0.05).Conclusion:The radial ultrasound small probe technology combined with RASSF1A and SHOX2 gene methylation detection can effectively improve the detection rate of peripheral solid/subsolid nodules in the lung,and provide a reference for early clinical screening.
