合成MRI、双指数及拉伸指数模型在鉴别前列腺癌与良性增生的应用OA

目的 比较合成磁共振成像(SyMRI)、双指数模型和拉伸指数模型(SEM)与单指数模型表现弥散系数(ADC)在前列腺癌(PCa)与良性前列腺增生(BPH)鉴别中的价值.方法 收集 2021 年 11 月至 2022 年 4 月于福建医科大学附属协和医院行前列腺MRI检查的 72 例患者,其中 40 例PCa、32 例BPH.比较PCa组与BPH组单指数ADC值,SyMRI的T1、T2、PD值,双指数的D、D*、f值,SEM的DDC、α值的差异,并分析各参数的诊断效果.构建SyMRI(T1+T2+PD)、双指数(D+D*+f)、SEM(DDC+α)等多参数模型,并与单指数ADC比较诊断效果.结果 PCa组ADC、T2、T1、D、D*、f、DDC值均低于BPH组,而PD与α在两组差异无统计学意义(P>0.05).其中SyMRI的T2(AUC=0.956)、双指数的D(AUC=0.959)和SEM的DDC(AUC=0.971)的诊断效能最高.SyMRI模型(T1+T2+PD)(AUC=0.973)和双指数模型(D+D*+f)(AUC=0.998)的诊断效能优于单指数模型(AUC=0.900),均为正改善,差异有统计学意义(P<0.05).SEM(DDC+α)的诊断效能(AUC=0.920)与单指数模型相比无显著提高.结论 T2、T1、D、D*、f、DDC能有效鉴别PCa和BPH.SyMRI和双指数多参数模型的诊断效果优于单指数ADC,可进一步推广应用.

Objective To compare the value of synthetic magnetic resonance imaging(SyMRI),bi-exponential model,and stretched exponential model(SEM)with mono-exponential model apparent diffusion coefficient(ADC)in differentiating prostate cancer(PCa)from benign prostatic hyperplasia(BPH).Methods This study enrolled 72 patients who underwent prostate MRI at Fujian Medical University Union Hospital between November 2021 and April 2022,including 40 cases of PCa and 32 cases of BPH.The differences of mono-exponential ADC values,T1,T2 and PD values of SyMRI,D,D* and f values of bi-exponential,DDC and α values of SEM were compared between PCa and BPH group,and the diagnostic efficacy of each parameter was analyzed.Multiparametric models,including SyMRI(T1+T2+PD),bi-exponential(D+D*+f),and SEM(DDC+α)were constructed and compared with mono-exponential ADC for diagnostic efficacy.Results ADC,T2,T1,D,D*,f and DDC values were lower in the PCa group than in the BPH group.However PD and α were not statistically different(P>0.05).The highest diagnostic efficacy was found in T2 of SyMRI(AUC=0.956),D of bi-exponential(AUC=0.959)and DDC of SEM(AUC=0.971).The diagnostic performance of SyMRI model(T1+T2+PD)(AUC=0.973)and bi-exponential model(D+D*+f)(AUC=0.998)was better than that of the mono-exponential model(AUC=0.900),both of which were positive improvements,with statistically significant differences(P<0.05).However,the diagnostic efficacy of SEM(DDC+α)(AUC=0.920)was not significantly improved compared with the mono-exponential model.Conclusion T2,T1,D,D*,f,and DDC can effectively identify PCa and BPH.The diagnostic efficacy of the multi-parameter models of SyMRI and bi-exponential were superior to that of mono-exponential ADC,which can be promoted for clinical use.
