单侧双通道与经椎板间窗单通道内镜技术治疗腰椎间盘突出症伴腰椎管狭窄症患者的效果比较OA
Comparison of the Effects of Unilateral Biportal and Interlaminar Uniportal Endoscopic Techniques in the Treatment of Patients with Lumbar Disc Herniation and Lumbar Spinal Stenosis
目的:对比单侧双通道内镜(UBE)与经椎板间窗单通道内镜(IUE)技术治疗腰椎间盘突出症伴腰椎狭窄症患者的临床效果.方法:回顾性分析 2022年 5月—2024年 10月博罗县人民医院收治的 68例腰椎间盘突出症合并腰椎管狭窄症患者资料,按照手术方法的不同将其分为对照组和研究组,各 34例.对照组采用IUE技术治疗,研究组采用UBE技术治疗.对比两组治疗效果、手术相关指标、腰部疼痛程度、腰椎功能及并发症发生情况.结果:研究组的治疗总有效率为100.00%,高于对照组的 79.41%,差异有统计学意义(P<0.05).研究组手术时间较对照组长,术中透视次数较对照组少,差异均有统计学意义(P<0.05).术后 5 d,研究组疼痛数字评分法评分低于对照组,差异有统计学意义(P<0.05).术后 3个月,研究组Oswestry功能障碍指数评分低于对照组,差异有统计学意义(P<0.05).研究组的并发症发生率为 2.94%,与对照组的 11.76%相比,差异无统计学意义(P>0.05).结论:UBE技术治疗腰椎间盘突出伴腰椎狭窄症,能够有效缓解患者术后腰部疼痛程度,提升其腰椎功能,降低并发症发生率,临床疗效确切,具有一定临床应用价值.
Objective:To compare the clinical efficacy of unilateral biportal endoscopy(UBE)technique and interlaminar uniportal endoscopic(IUE)techniques in the treatment of patients with lumbar disc herniation with lumbar spinal stenosis.Methods:A retrospective analysis was conducted on the data of 68 patients with lumbar disc herniation combined with lumbar spinal stenosis admitted to Boluo County People's Hospital from May 2022 to October 2024.They were divided into a control group and a study group according to the different surgical methods,with 34 cases in each group.The control group was treated with IUE technology,while the study group was treated with UBE technology.The therapeutic effects,surgery-related indicators,degree of lumbar pain,lumbar function and occurrence of complications of the two groups were compared.Results:The total effective rate of treatment in the study group was 100.00%,which was higher than 79.41%in the control group,the difference was statistically significant(P<0.05).The operation time of the study group was longer than that of the control group,the number of intraoperative fluoroscopy sessions was less than that of the control group,the difference were statistically significant(P<0.05).Five days after the operation,the score of the Numerical Rating Scale for Pain in the study group was lower than that in the control group,and difference was statistically significant(P<0.05).Three months after the operation,the Oswestry Disability Index score of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the study group was 2.94%,compared with 11.76%in the control group,the difference was not statistically significant(P>0.05).Conclusion:The UBE technique in the treatment of lumbar intervertebral disc protrusion with lumbar stenosis can effectively relieve the degree of postoperative lumbar pain in patients,improve their lumbar function,and has a low incidence of complications.The clinical efficacy is definite and it has clinical application value.
