克氏针联合髓内内固定术对掌骨颈骨折患者治疗效果及手功能的影响分析OA
Analysis of the Therapeutic Effect and Hand Function of Kirschner wire Combined with Intramedullary Internal Fixation in the Treatment of Metacarpal Neck Fractures
目的:探讨克氏针联合髓内固定术对掌骨颈骨折患者的临床治疗效果及对手功能的影响.方法:回顾性分析 2021年 6月—2024年 5月利津县中医院收治的 60例掌骨颈骨折患者的临床资料,按照手术方法的不同将其分为对照组(n=25)和研究组(n=35).对照组采用切开复位克氏针固定术治疗,研究组采用克氏针联合髓内内固定术治疗.对比两组临床指标、腕掌关节功能、手功能及并发症发生情况.结果:研究组术中出血量少于对照组、骨折临床愈合时间短于对照组,差异均有统计学意义(P<0.05);两组手术时间对比,差异无统计学意义(P>0.05).研究组腕掌关节功能优良率为97.14%,高于对照组的76.00%,差异有统计学意义(P<0.05).术后 6周,研究组Carroll手功能评定量表评分高于对照组,差异有统计学意义(P<0.05).研究组并发症发生率为2.86%,低于对照组的16.00%,差异有统计学意义(P<0.05).结论:克氏针联合髓内内固定术治疗掌骨颈骨折患者可减少术中出血量、缩短临床愈合时间,更有利于促进腕掌关节功能及手功能恢复,且术后并发症更少.
Objective:To explore the clinical therapeutic effect of Kirschner wire combined with intramedullary fixation on patients with metacarpal neck fractures and its influence on hand function.Methods:A retrospective analysis was conducted on the clinical data of 60 patients with metacarpal neck fractures admitted to Lijin County Hospital of Traditional Chinese Medicine from June 2021 to May 2024.They were divided into a control group(n=25)and a study group(n=35)according to different surgical methods.The control group was treated with open reduction and Kirschner wire fixation,while the study group was treated with Kirschner wire combined with intramedullary internal fixation.The clinical indicators,carpal and palmar joint functions,hand functions and the occurrence of complications of the two groups were compared.Results:The intraoperative blood loss in the study group was less than that in the control group,and the clinical healing time of fractures was shorter than that in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the operation time between the two groups(P>0.05).The excellent and good rate of carpal and metacarpal joint function in the study group was 97.14%,which was higher than 76.00%in the control group,the difference was statistically significant(P<0.05).Six weeks after the operation,the score of the Carroll Hand Function Assessment Scale in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 2.86%,which was lower than 16.00%in the control group,the difference was statistically significant(P<0.05).Conclusion:Kirschner wire combined with intramedullary internal fixation in the treatment of patients with metacarpal neck fractures can reduce intraoperative blood loss,shorten the clinical healing time,be more conducive to promoting the recovery of carpal and metacarpal joint function and hand function,and have fewer postoperative complications.
