目的 探讨田七元胡胶囊在骨折疼痛管理中的作用及其对COX2-PGE2-cAMP信号通路的调节机制,为该中成药制剂在临床应用的镇痛效果提供科学依据.方法 采用胫骨骨折SD大鼠模型,将 24 只大鼠随机分为假手术组、模型组、田七元胡组和塞来昔布组,每组各 6 只.假手术组和模型组予 0.9%生理盐水灌胃,田七元胡组和塞来昔布组分别灌胃 108 mg/kg的田七元胡溶液和 0.2 g/kg的塞来昔布溶液.术后第 1 天和第7 天通过酶联免疫吸附试验(ELISA)检测大鼠血清前列腺素E2(PGE2)和环磷酸腺苷(cAMP)水平;采用蛋白质印迹(WB)检测胫骨旁周围组织中环氧化酶(COX)1 和COX2 蛋白表达.结果 术后第 1 天,模型组、田七元胡组、塞来昔布组的血清PGE2 和cAMP水平高于假手术组,差异有统计学意义(P<0.05);模型组和田七元胡组、田七元胡组和塞来昔布组、模型组和塞来昔布组的血清PGE2 和cAMP水平比较,差异无统计学意义(P>0.05).术后第 7 天,田七元胡组和塞来昔布组的PGE2 和cAMP水平均低于模型组,差异有统计学意义(P<0.05);田七元胡组和塞来昔布组的PGE2 和cAMP水平比较,差异无统计学意义(P>0.05).WB结果显示,与第 1 天比较,田七元胡组在第 7 天骨折周围组织中COX1 和COX2 的表达显著下调,差异有统计学意义(P<0.05);术后第 7 天,田七元胡组大鼠胫骨旁组织的COX1 和COX2 表达水平低于模型组,差异有统计学意义(P<0.05).结论 田七元胡胶囊通过调节COX2-PGE2-cAMP通路,降低PGE2 和cAMP水平,并抑制COX1 和COX2 的表达,具有良好的镇痛效果.本研究为田七元胡胶囊的临床应用提供了新的实验依据.

Objective To explore the role of Tianqi Yuanhu capsule in fracture pain management and its regulatory mechanism on COX2-PGE2-cAMP signaling pathway,and to provide a scientific basis for the analgesic effect of this proprietary Chinese medicine preparation in clinical application.Methods The tibial fracture model of SD rats was used,and 24 rats were randomly divided into the sham-operation group,the model group,the Tianqi Yuanhu group and the celecoxib group,with 6 rats in each group.The sham-operation group and model group were gavaged with 0.9%saline,while the Tianqi Yuanhu group and celecoxib group were gavaged with 108 mg/kg of Tianqi Yuanhu and 0.2 g/kg of celecoxib,respectively.The serum levels of prostaglandin E2(PGE2)and cyclic adenosine monophosphate(cAMP)were measured by enzyme-linked immunosorbent assay(ELISA)on the first and seventh postoperative days,and the protein expression of cyclooxygenase(COX)-1 and COX2 was detected by protein blotting(WB)in the peripheral tissues of the tibial parietal.Results On the first postoperative day,the serum PGE2 and cAMP levels in the model group,Tianqi yuanhu group,and celecoxib group were higher than those in the sham-operated group,and the differences were statistically significant(P<0.05);there was no statistically significant difference in the comparison of serum PGE2 and cAMP levels between the model group and the Tianqi yuanhu group,between the Tianqi yuanhu group and the celecoxib group,and between the model group and the celecoxib group(P>0.05).On the seventh postoperative day,the levels of PGE2 and cAMP were lower in the Tianqi yuanhu group and the celecoxib group than in the model group,and the difference was statistically significant(P<0.05);the comparison of the levels of PGE2 and cAMP between the Tianqi yuanhu group and the celecoxib group showed no statistically significant differences(P>0.05).The results of the WB showed that,compared with the first day,in the tissues surrounding the fracture in the Tianqi Yuanhu group on the seventh day,the expression of COX1 and COX2 was significantly down-regulated,and the difference was statistically significant(P<0.05);on the seventh day after surgery,the expression levels of COX1 and COX2 in the parietal tissues of the tibia in the Tianqi Yuanhu group were lower than those in the model group,and the difference was statistically significant(P<0.05).Conclusion Tianqi Yuanhu Capsule alleviates post-fracture pain by regulating the COX2-PGE2-cAMP pathway,reducing PGE2,cAMP,and COX1/COX2 levels.These findings support its clinical potential.
