不同参数脉冲射频对腰椎间盘突出症疼痛患者的疗效差异研究OA
Efficacy Difference Study of Pulsed Radio Frequency with Different Parameters in Patients with Pain Caused by Lumbar Disc Herniation
目的:探讨不同参数脉冲射频对腰椎间盘突出症疼痛患者的疗效差异.方法:选取 2020年 4月—2024年 4月阜新市中心医院收治的72例腰椎间盘突出症疼痛患者为研究对象,按随机数字表法将其分为A、B、C组,各24例.A组设置脉冲射频参数为频率2 Hz,脉冲宽度20 ms,温度42℃,治疗时间300 s;B组设置参数为频率3 Hz,脉冲宽度 30 ms,温度 45℃,治疗时间 600 s;C组设置参数为频率 4 Hz,脉冲宽度 40 ms,温度 50℃,治疗时间900 s.对比三组疼痛程度、腰椎功能、腰椎活动度、炎症因子水平及不良反应发生情况.结果:治疗后 1周、1、3个月,三组疼痛视觉模拟法评分均较治疗前降低,且C组均低于A、B组,B组均低于A组差异均有统计学意义(P<0.05).治疗后 1、3个月,三组Oswestry功能障碍指数评分均较治疗前降低,且C组均低于A、B组,B组均低于A组,差异均有统计学意义(P<0.05).治疗后 3个月,三组腰椎前屈、后伸、侧屈活动度均较治疗前增大,且C组均大于A、B组,B组均低于A组,差异均有统计学意义(P<0.05).治疗后1周,三组白细胞介素-6、肿瘤坏死因子-α均较治疗前降低,且C组均低于A、B组,B组均低于A组,差异均有统计学意义(P<0.05).A、B、C组不良反应发生率分别为 12.50%、20.80%、33.33%,但三组比较,差异无统计学意义(P>0.05).结论:不同参数脉冲射频对腰椎间盘突出症疼痛患者的疗效存在差异,高频脉冲射频在缓解疼痛、恢复腰椎功能及抑制炎症方面效果更优,但不良反应发生率较高.
Objective:To explore the differences in the efficacy of pulsed radio frequency with different parameters in patients with pain caused by lumbar disc herniation Methods:A total of 72 patients with pain caused by lumbar disc herniation admitted to Fuxin Central Hospital from April 2020 to April 2024 were selected as the study objects,and were divided into group A,B,and C by random number table method,with 24 cases in each group.The pulsed radio frequency parameters of group A were frequency 2 Hz,pulse width 20 ms,temperature 42℃,and treatment time 300 s;the parameters of group B were frequency 3 Hz,pulse width 30 ms,temperature 45℃,and treatment time 600 s;the parameters of group C were frequency 4 Hz,pulse width 40 ms,temperature 50℃,and treatment time 900 s.The pain level,lumbar spine function,lumbar spine mobility,inflammatory levels factors and the occurrence of adverse reactions were compared between the three groups.Results:At 1 week,1 and 3 months after treatment,the Pain Visual Analogue Scale scores of the three groups were lower than those before treatment,and the group C were lower than the groups A and B,the difference were statistically significant(P<0.05).At 1 and 3 months after treatment,the scores of Oswestry Disability Index in the three groups were lower than those before treatment,and the group C were lower than the groups A and B,the group B were lower than the Group A,the difference were statistically significant(P<0.05).Three months after treatment,the range of motion of lumbar spine forward flexion,posterior extension and lateral flexion in the three groups were increased than those before treatment,and group C were larger than the groups A and B,the group B were larger than the Group A,the difference were statistically significant(P<0.05).One week after the treatment,interleukin-6 and tumor necrosis factor-α in all three groups decreased compared with those before the treatment,the group C were lower than the groups A and B,and the group B were lower than the group A,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the groups A,B and C were 12.50%,20.80%and 33.33%,respectively,but there was no significant difference between the three groups(P>0.05).Conclusion:There are significant differences in the efficacy of pulsed radiofrequency treatment for patients with pain caused by lumbar disc herniation with different parameters,high-frequency pulsed radiofrequency has better effects in pain relief,lumbar spine function recovery,and inflammation inhibition,but the incidence of adverse reactions was relatively high.
