股骨颈动力交叉钉系统内固定术与空心钉内固定术在治疗移位股骨颈骨折中的效果对比OA
Comparison of the Effects of Femoral Neck Power Cross-Nailing System Internal Fixation with Hollow Nail Internal Fixation in the Treatment of Displaced Femoral Neck Fractures
目的:比较股骨颈动力交叉钉系统内固定术与空心钉内固定术在治疗移位股骨颈骨折患者中的效果.方法:选取 2022年 1月—2024年 6月合肥市骨科医院收治的 100例移位股骨颈骨折患者为研究对象,采用随机数字表法将其分为研究组(n=40)和对照组(n=60).对照组采用空心钉内固定术治疗,研究组采用股骨颈动力交叉钉系统内固定术治疗.比较两组手术及术后康复指标、疼痛程度、髋关节屈伸活动度、髋关节功能、并发症发生情况.结果:研究组术中出血量少于对照组,切口长度、手术时间、骨折愈合时间均短于对照组,差异有统计学意义(P<0.05).术后 3、5 d,两组疼痛视觉模拟法评分均低于术前,且术后 5 d研究组低于对照组,差异均有统计学意义(P<0.05).术后 6个月,两组髋关节屈伸活动度均大于术后 1个月,且研究组高于对照组,差异均有统计学意义(P<0.05).术后 1、3、6个月,两组Harris髋关节功能评分量表评分均高于术前,且研究组均高于对照组,差异均有统计学意义(P<0.05).研究组并发症发生率为 0.00%,低于对照组的 13.33%,差异有统计学意义(P<0.05).结论:相较于空心钉内固定术,股骨颈动力交叉钉系统内固定术在移位股骨颈骨折治疗中可有效减少患者术中出血量、缩短手术及骨折愈合时间、降低术后疼痛程度,有利于髋关节屈伸活动度改善及功能恢复,且并发症发生率低.
Objective:To compare the effects of femoral neck power cross-nailing system internal fixation and hollow nail internal fixation in the treatment of patients with displaced femoral neck fracture.Methods:A total of 100 patients with displaced femoral neck fracture admitted to Hefei Orthopaedic Hospital from January 2022 to June 2024 were selected as the study objects,and were divided into a control group(n=60)and a study group(n=40)by random number table method.The control group were treated with hollow nail internal fixation,and the study group were treated with femoral neck power cross-nailing system internal fixation.The surgical and postoperative rehabilitation indexes,pain degree,hip flexion and extension range of motion,hip joint function,and occurrence of complications were compared between the two groups.Results:The intraoperative blood loss in the study group was lower than that in the control group,and the incision length,operation time and fracture healing time were shorter than those in the control group,the difference were statistically significant(P<0.05).At 3 and 5 days after surgery,the scores of the Pain Visual Analogue Scoring in both groups were lower than those before operation,and on the 5 day after surgery;the study group was lower than the control group,the difference were statistically significant(P<0.05).At 6 months after surgery,the range of motion of hip flexion and extension in both groups were bigger than those at 1 month after surgery,and the study group was higher than the control group,the difference were statistically significant(P<0.05).At 1,3 and 6 months after surgery,the scores of Harris Hip Function Scale in both groups were higher than those before operation,and the study group were higher than the control group,the differences were statistically significant(P<0.05).The complication rate of the study group was 0.00%,which was lower than 13.33%of the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with hollow nail internal fixation,femoral neck dynamic cross nail system internal fixation can effectively reduce the amount of intraoperative blood loss,shorten the time of surgery and fracture healing,reduce the postoperative pain degree,and facilitate the improvement of hip flexion range of motion and the recovery of extension function in the treatment of displaced femoral neck fractures,and the complication rate is low.
