单通道与单侧双通道脊柱内镜手术治疗腰椎管狭窄症患者的效果对比研究OA
Comparative Study of the Effect of Single Channel and Unilateral Double Channel Spinal Endoscopy Surgery on Patients with Lumbar Spinal Stenosis
目的:探讨并比较单侧双通道脊柱内镜手术与单通道脊柱内镜手术治疗腰椎管狭窄症(LSS)患者的临床效果.方法:选取 2022年 3月—2024年 2月永州市中心医院收治的 94例LSS患者为研究对象,依据随机数字表法将其分为对照组与研究组,各 47例.研究组采用单侧双通道脊柱内镜手术治疗,对照组采用单通道脊柱内镜手术治疗.比较两组治疗效果、腰椎功能、手术指标、生活质量.结果:研究组治疗总有效率为 95.74%,高于对照组的 82.97%,差异有统计学意义(P<0.05).术后 12周,研究组腰椎日本骨科协会评分中各个维度评分均高于对照组,差异均有统计学意义(P<0.05).研究组手术时间长于对照组,术中出血量低于对照组,术后首次下床活动时间及住院时间均短于对照组,差异均有统计学意义(P<0.05).术后 12周,两组健康调查简表中各个维度评分均高于术前,且研究组均高于对照组,差异均有统计学意义(P<0.05).结论:单侧双通道脊柱内镜手术相较于单通道脊柱内镜手术能够有效改善LSS患者的腰椎功能和生活质量,且手术创伤小,利于患者术后恢复,具有较好的临床治疗效果.
Objective:To investigate and compare the clinical effects of unilateral double channel spinal endoscopy surgery and unilateral single channel spinal endoscopy surgery in the treatment of patients with lumbar spinal stenosis(LSS).Methods:A total of 94 LSS patients admitted to Yongzhou Central Hospital from March 2022 to February 2024 were selected as the study objects,and were divided into a control group and a study group according to random number table method,with 47 cases in each group.The study group was treated with unilateral double channel spinal endoscopy surgery,while the control group was treated with single channel spinal endoscopy surgery.The treatment effect,lumbar function,surgical indexes and quality of life were compared between the two groups.Results:The total effective rate of the study group was 95.74%,which was higher than 82.97%of the control group,the difference was statistically significant(P<0.05).At 12 weeks after surgery,the all dimensions in the Lumbar Japanese Orthopaedic Association scores in the study group were higher than those in the control group,with statistical significance(P<0.05).The operation time of the study group was longer than that of the control group,the amount of intraoperative blood loss was lower than that of the control group,and the first time of getting out of bed after surgery and the length of hospitalization were shorter than those of the control group,the differences were statistically significant(P<0.05).At 12 weeks after surgery,the scores of all dimensions in the MOS Item Short Form Health Survey in both groups were higher than those before surgery,and the study group were higher than the control group,with statistical significance(P<0.05).Conclusion:Compared with single channel spinal endoscopy,surgery unilateral double channel spinal endoscopy surgery can effectively improve the lumbar function and quality of life of CSS patients,and the surgical trauma is less,which is conducive to postoperative recovery of patients,and has a better clinical therapeutic effect.
