针灸治疗偏头痛作用机制研究进展OA

针灸治疗偏头痛的机制主要包括调节脑部血流速度、影响神经递质释放、抑制炎性反应、调节中枢镇痛机制、抑制神经元激活、抑制胶质细胞活化、促进阿片类受体、调节脑-肠-菌轴等.目前,针灸治疗偏头痛临床研究逐渐丰富,但仍存在以下问题:治法相对单一,主要集中在针刺研究方向;没有区别证型,未发挥最大疗效;缺乏对机制之间联系及整体的研究等.今后,可加强对灸法治疗偏头痛机制的研究,促进临床治疗多样性,充分发挥针灸治疗偏头痛的作用;研究针灸治疗不同证型偏头痛机制的区别,充分考虑患者个体差异性,提高临床疗效;进一步挖掘不同机制之间的内在联系,促进针灸治疗偏头痛整体机制研究的发展.

The mechanism of acupuncture and moxibustion in treating migraine mainly includes regulating cerebral blood flow velocity,affecting neurotransmitter release,inhibiting inflammatory response,regulating central analgesia mechanism,inhibiting neuronal activa-tion,inhibiting glial cell activation,promoting opioid receptors,and regulating brain-gut-bacterial axis.At present,the clinical re-search on acupuncture and moxibustion treatment of migraine is gradually enriched,but there are still the following problems:The treat-ment is relatively simple,mainly focusing on acupuncture;There is no differentiation in syndrome types and the maximum therapeutic effect has not been achieved;lack of research on the connections between mechanisms and the overall situation.In the future,it is neces-sary to strengthen the research on the mechanism of moxibustion treatment of migraine,promote the diversity of clinical treatment,and maximize the efficacy of acupuncture and moxibustion;to study whether the mechanism of acupuncture and moxibustion in treating mi-graine of different syndromes is different,fully consider the individual differences of patients,and improve the clinical efficacy;to further explore the internal relationship between different mechanisms,and promote the development of the research on the overall mechanism of acupuncture and moxibustion treatment of migraine.
