不同剂量珂立苏治疗重症新生儿呼吸窘迫综合征的对照研究OA

A comparative study of different doses of Calsurf in the treatment of severe neonatal respiratory distress syndrome

目的 探讨不同剂量珂立苏治疗重症新生儿呼吸窘迫综合征(NRDS)的价值.方法 将 2020 年 1 月至 2023 年 6 月在廉江市人民医院新生儿重症监护室医治的 126 例重症NRDS患儿按照随机数表法分为A、B、C组.A、B、C组分别给予 70、100、140 mg/kg珂立苏.比较三组的血气指标、治疗情况、住院费用、住院时间及并发症.结果 与A组比较,B组和C组治疗 6 h后的动脉二氧化碳分压、珂立苏用药次数≥2 构成比、化验费、治疗费和护理费均明显更低,治疗 6 h后的动脉血氧分压和氧合指数均明显更高,辅助通气时间、氧疗时间和总住院时间均明显更短,差异有统计学意义(P<0.05).药物费和总住院费以B组最低,C组次之,A组最高,差异有统计学意义(P<0.05).结论 100 mg/kg珂立苏治疗重症NRDS可降低药物费和总住院费.100、140 mg/kg珂立苏治疗NRDS在改善血气指标,减少珂立苏用药次数,缩短辅助通气时间、氧疗时间和住院时间方面均优于 70 mg/kg珂立苏.

Objective To explore the value of different doses of Calsurf in the treatment of severe neonatal respiratory distress syndrome(NRDS).Methods A total of 126 children with severe NRDS treated in the neonatal intensive care unit of Lianjiang People's Hospital from January 2020 to June 2023 were divided into groups A,B,and C by random table method.Groups A,B,and C were given 70 mg/kg,100 mg/kg,and 140 mg/kg of Calsurf,respectively.The blood gas indicators,treatment status,hospitalization costs,length of stay,and complications among the three groups were compared.Results Compared with Group A,Group B and Group C had significantly lower arterial carbon dioxide partial pressure,a ratio of≥2 times of use of Calsurf,laboratory expenses,treatment expenses,and nursing expenses after 6 hours of treatment.The arterial oxygen partial pressure and oxygenation index were significantly higher after 6 hours of treatment,and the time for auxiliary ventilation,oxygen therapy,and total hospitalization were significantly shorter,with statistically significant differences(P<0.05).The drug and total hospitalization expenses were the lowest in Group B,followed by Group C,and the highest in Group A,with statistically significant differences(P<0.05).Conclusion 100 mg/kg Calsurf can reduce medication costs and total hospitalization expenses in the treatment of severe NRDS.The treatment of NRDS with 100 mg/kg and 140 mg/kg Calsurf is superior to 70 mg/kg Calsurf in improving blood gas indicators,reducing the frequency of Calsurf medication,shortening auxiliary ventilation time,oxygen therapy time,and hospital stay.
